Abstract

Gubitak zuba narušava harmoničnu funkciju stomatognatog sustava. Dugotrajna djelomična ili potpuna bezubost rezultira teškom atrofijom alveolne kosti, što znatno otežava implantoprotetsku terapiju. Paulo Maló razradio je začetnu ideju slavnog profesora Brånemarka te patentirao revolucionaran fiksni implantoprotetski terapijski koncept za pacijente s ograničenom količinom preostale alveolne kosti – koncept All-on-4/6. Koncept je prepoznat u svijetu te ga je registrirala globalna kompanija Nobel Biocare. Među brojnim prednostima takvoga terapijskog pristupa najvažnije su: ugradnja dentalnih implantata uz zaobilaženje kritičnih anatomskih struktura, minimalan broj dentalnih implantata, stvaranje uvjeta za imedijatno opterećenje, izbjegavanje augmentacijskih postupaka, fiksna protetska terapija, vijčana veza, cjelovita oralna rehabilitacija bezubih pacijenata u jednom danu, kraći postoperativni tijek, poboljšanje kvalitete života te predvidivost i sigurnost liječenja potvrđena brojnim višegodišnjim praćenjima. Cilj je rada sistematično prikazati planiranje terapije, kirurški te protetski protokol izrade fiksnoprotetskog nadomjeska retiniranog vijkom s imedijatnim opterećenjem.

Abstract (english)

Tooth loss disrupts the harmonic function of the stomatognathic system. Long-lasting partial or complete edentulism results in a severe atrophy of the alveolar bone, which complicates implantoprosthetic therapy significantly. Paulo Maló developed the initial idea of the famous professor Brånemark and patented the revolutionary fixed implantoprosthetic therapeutic concept for patients with a limited amount of remaining alveolar bone – the All-on-4/6 Concept. The concept is recognized worldwide and registered by the global company of Nobel Biocare. Of the many advantages of such therapeutic approach, the most important is insertion of dental implants around a dental critical anatomic structure, minimal number of dental implants, creation of conditions for immediate loading, avoidance of augmentation procedures, fixed prosthetic therapy, screw connection, complete oral rehabilitation of edentulous patients in one day, shorter postoperative recovery, improving the quality of life and predictability and safety of treatment confirmed by many years of follow-up. The aim of this paper is to provide a systematic overview of the therapy planning, surgical and prosthetic protocols for the production of screw retained fixed prosthesis with immediate loading.
