| Abstract | Implantoprotetska rehabilitacija danas se smatra uspješnom i pouzdanom metodom sanacije
djelomične ili potpune bezubosti. Titanski implantati odlikuju se svojstvima trajnosti, dobre
biokompatibilnosti i mogućnošću oseointegracije i danas su još uvijek prvi izbor prilikom
odabira implantološkog sustava. Međutim, zbog povećanih estetskih zahtjeva pacijenata, a i
nepovoljnih svojstava jače adherencije bakterijskog plaka te posljedičnih upalnih odgovora,
keramika je na tržištu predstavljena kao alternativni bezmetalni materijal za izradu implantata,
dobrih estetskih i mehaničkih svojstava te izrazite biokompatibilnosti i osteokonduktivnosti.
Odlična oseointegracija postiže se modifikacijom površine keramike, različitim mehaničkim ili
kemijskim procesima, od kojih se najčešće koristi pjeskarenje sa selektivnim jetkanjem
kiselinom. Najčešći keramički materijal za izradu implantata je polikristalni cirkonijev dioksid
ojačan itrijem. Nadomjesci iz ovog materijala se izrađuju CAD/CAM tehnologijom.
Implantološki sustav sastoji se od tijela implantata, nadogradnje implantata te cementirane ili
vijkom pričvršćene pripadajuće protetske suprastrukture. Zavisno o odnosu tijela i nadogradnje,
implantate dijelimo na jednodijelne i dvodijelne. Kod jednodijelnih su tijelo i nadogradnja
spojeni u procesu izrade, dok kod dvodijelnih dolaze kao zasebne komponente te se spajaju
odgovarajućim vijkom. Zbog mogućnosti djelovanja nepoželjnih kosih i horizontalnih sila,
različitim antirotacijskim i retencijskim elementima, te najčešće unutarnjim heksagonalnim
spojem sprječava se rotacija i stvaranje mikropropuštanja. Individualne keramičke implantatne
nadogradnje često se koriste u prednjem dijelu zubnog luka, zbog prirodnog izgleda i boje
mekog tkiva. Različita klinička istraživanja potvrđuju odlične rezultate keramičkih implantata
nakon opterećenja u trajanju do pet godina. Kako su za kliničku upotrebu poprilično nov
materijal, malo je podataka o dugoročnim praćenjima za periode dulje od deset godina. Kao
najčešći razlozi neuspjeha terapije navode se aseptično popuštanje, mehaničke i tehnološke
komplikacije, ponajprije zbog fenomena „starenja keramike“, ali bez bioloških oštećenja i
razvoja perimukozitisa i periimplantitisa. |
| Abstract (english) | Implantoprosthetic rehabilitation is considered nowadays to be a successful and reliable method
for rehabilitating partial or complete toothlessness. Titanium implants are characterized by
durability, good biocompatibility and the possibility of osseointegration, and are still regarded
as the first choice upon choosing an implantological system. However, due to the increased
aesthetic demands of patients, as well as the unfavorable properties of stronger adhesion of
bacterial plaque and consequent inflammatory responses, ceramics have been presented on the
market as an alternative, metal-free material for making implants, with good aesthetic and
mechanical properties, as well as strong biocompatibility and osteoconductivity. Excellent
osseointegration is achieved by modifying the surface of ceramics, using different mechanical
or chemical processes, the most common of which is sandblasting with selective acid etching.
The most common ceramic material for making implants is polycrystalline zirconium dioxide
reinforced with yttrium. Protheses out of this material are made using CAD/CAM technology.
Implant system consists of the implant body, abutment and cemented or screw-attached
prosthetic suprastructure. Depending on the relationship between the body and the abutment we
divide implants into one-piece and two-piece systems. With one-piece, the body and the
abutment are connected in the manufacturing process, while with two-piece, they come as
separate components and are connected with a suitable screw. Due to the possibility of
unwanted oblique and horizontal forces, various anti-rotational and retention elements, usually
an internal hexagonal joint are used to prevent rotation and the creation of microleakage.
Individual ceramic implant abutments are often used in the front region, due to the natural
appearance and color of the soft tissue. Various clinical studies confirm the excellent results of
ceramic implants after loading for up to 5 years. As they are relatively new material for clinical
use, there is little dana on long-term follow-up for periods longer than 10 years. The most
common reasons for implant failure are aseptic loosening, mechanical and technological
complications, primarily due to the phenomenon of ceramic „ageing“, but without biological
damage and the development of perimucositis and periimplantitis. |
