Abstract

Razvoj digitalne tehnologije omogućio je brzi napredak dentalne medicine. Razvoj CAD/CAM sustava i 3D pisača ubrzava rad doktora dentalne medicine, smanjuje broj dolazaka pacijenata, te je ubrzan i cjelokupni proces izrade implantoprotetskog nadomjeska. Postupak izvedbe počinje uzimanjem optičkoga otiska intraoralnim skenerom ili skeniranjem sadrenoga modela. Nakon dizajniranja u CAD softveru, jedinica za glodanje ili 3D pisač na temelju zadanih parametara izrađuju željeni nadomjestak. Materijali koji se koriste za obradu CAD/CAM tehnologijom dolaze u obliku blokova ili diskova, a ovisno o indikaciji odnosno specifičnim estetskim ili funkcionalnim potrebama moguće je izraditi brojne konstrukcije poput ljuskica, inlaya, onlaya, pojedinačnih krunica, višečlane mosne konstrukcije, uključujući i teleskopske krunice, prečke i totalne proteze. CAD/CAM sustav neposredne izrade u ordinaciji omogućava da pacijent u jednome danu dobije fiksnoprotetski nadomjestak. Za izradu imedijatnih protetskih nadomjestaka na implantatima potrebno je prethodno odrediti njihov položaj u čeljusti stoga se u posljednje vrijeme u implantologiji sve više upotrebljavaju kirurški predlošci u potpunosti izvedeni računalnom tehnologijom. Planiranje postavljanja implantata izvodi se uz pomoć CAD softvera, CBCT i intraoralne snimke, a sam kirurški predložak izrađuje se tehnikom glodanja, SLA (eng. stereolithography) 3D printanjem ili FDM (eng. fused deposition modeling) 3D printanjem. Uporaba 3D pisača u protetici uključuje izradu individualnih žlica za otiske, situacijskih modela, privremenih nadomjestaka, metalnih osnova za krunice i mostove, konstrukcijskih modela za lijevanje od polimera ili izravno metalne baze proteza, akrilatnih proteza te udlaga.

Abstract (english)

The development of digital technology has enabled a fast advancement of dental medicine. The development of CAD/CAM systems and 3D printers accelerates the work of dental medicine doctors, lowers the number of patient appointments and makes the whole process of the making of the implant-prosthetic replacement faster. The procedure starts by taking an optical impression with an intraoral scanner or by scanning a plaster model. After designing it in CAD software, the milling cutter or the 3D printer create the desired implant from the given paramenters. The materials used for the CAD/CAM technology elaboration come in the form of blocks or discs and depending on the indication or specific esthetic or functional needs, it is possible to make numerous constructions like veneers, inlays, onlays, individual crowns, multiple bridge construction, including telescopic crowns, bars and complete denture. The CAD/CAM system of direct making in the dental clinic enables the patient to receive a fixed prosthetic replacement in one day. For the making of immediate prosthetic replacements on implants it is necessary to previously determine their position in the jaw. For that reason surgical templates created with computerized technology are lately increasingly used in implantology. Implant location planning is performed with CAD software, CBCT and intraoral imaging while the surgical template is made by use of a milling cutter, SLA (stereolitography) or 3D printing or FDM (fused deposition modeling) 3D printing. The use of 3D printers in prosthetics includes the making of individual impression spoons, situation models, temporary replacements, metal bases for crowns and bridges, construction models for polymer casting or directly the metal base of dentures, acrylate dentures and splints.
