Abstract

Nekarijesna oštećenja tvrdih zubnih tkiva nastaju pod utjecajem fizičko-mehaničkih i kemijskih čimbenika tijekom života. Ne povezuju se s bakterijskim uzročnicima te imaju multifaktorijalnu etiologiju. Trošenje zubi nastaje kao rezultat tri procesa: abrazije (interakcijom zuba i stranih tijela), atricije (kontakt zub o zub) te erozije (kemijska disolucija zubne supstance). Abfrakcija nastaje zbog abnormalnog žvačnog opterećenja u području vrata zuba. Navedeni mehanizmi rijetko djeluju zasebno. Međusobno su isprepleteni te se javljaju istovremeno pod utjecajem različitih egzogenih i endogenih uzročnika. Rana dijagnostika i prevencija bitne su u prepoznavanju i zaustavljanju daljnje progresije lezija i uklanjanja rizičnih čimbenika. Definitivna terapija nekarijesnih lezija uključuje minimalno invazivne restorativne postupke ili protetska rješenja u svrhu očuvanja vitaliteta i integriteta pojedinih zubi te estetskog i funkcijskog zbrinjavanja pacijenta. Njihov odabir ovisi o etiologiji oštećenja, simptomima, dobi, željama i financijskim mogućnostima pacijenta te mogućnosti korištenja pojedinih tehnika s obzirom na izgled, smještaj i opsežnost lezija.

Abstract (english)

Non-carious lesions of dental hard tissues are caused by physical-mechanical and chemical factors during a lifetime. They are not associated with bacterial agents and have multifactorial etiology. Tooth wear occurs as a result of three processes: abrasion (the interaction of teeth and foreign bodies), attrition (tooth-to-tooth contact) and erosion (chemical dissolution of dental substance). Abfraction is caused by abnormal occlusal load in the cervical area of the tooth. These mechanisms rarely act separately. They are intertwined and occur simultaneously under the influence of various exogenous and endogenous agents. Early diagnosis and prevention are essential in identifying and stopping further lesion progression and elimination of risk factors. Definitive therapy for non-carious lesions involves minimally invasive restorative procedures or prosthetic solutions in order to preserve the vitality and integrity of an individual tooth, and aesthetic and functional care for the patient. Their selection depends on the etiology of the lesion, the symptoms, age, preferences and financial situation of the patient and the ability to use certain techniques considering the appearance, location and the extent of the lesions.
