Abstract

Kontrola boli preduvjet je kooperativnosti pacijenta, osobito u dječjoj dentalnoj medicini. Prevencija boli ujedno znači i prevenciju anksioznosti, straha i fobije koji su često uzrokovani nelagodom i boli proživljenima tijekom stomatoloških zahvata. Danas je u dentalnoj medicini moguće gotovo sve zahvate učiniti bezbolnim, jer je na raspolaganju velik niz visoko potentnih i slabo toksičnih anestetika. Ipak, sam ubod injekcijskom iglom radi primjene lokalne anestezije, smatra se najčešćim uzrokom boli, a istovremeno je i jedini način na koji se može otkloniti bol koja je prisutna tijekom stomatoloških zahvata. Kako bi se umanjila nelagoda povezana s injekcijama, sredinom 1990.-ih godina razvijen je koncept računalno kontrolirane anestezije, takozvani C-CLAD (eng., computer-controlled local anesthetic delivery). C-CLAD podrazumijeva primjenu klasičnih, suplementarnih ili modificiranih tehnika lokalne anestezije uz uporabu posebno dizajniranih računalnih sustava s ciljem minimaliziranja mogućnosti ljudske pogreške pri davanju anestezije. Pritom je takav način anestezije prigodan za primjenu kod djece jer su bol, strah i anksioznost reducirani u odnosu na primjenu klasičnih manualnih štrcaljki. Prvi tržištu predstavljeni C-CLAD sustav bio je The Wand™ (Milestone Scientific Inc., Livingston, NJ, SAD). Kasnije je isti proizvođač predstavio Wand Plus, CompuDent i STA (Single-Tooth-Anesthesia) sustave. Danas tržište nudi još nekolicinu C-CLAD-ova. Zajedničko tim sustavima jest kontrola brzine, protoka i tlaka prilikom aplikacije anestetičke otopine te redukcija stresa, kako u pacijenta, tako i u operatera. Ipak, takvi su sustavi skupi i zahtijevaju dodatno vrijeme i opremu, kao i operaterovo poznavanje rukovanja uređajem.

Abstract (english)

Pain control is a precondition for patient’s cooperation, especially in pediatric dentistry. The pain prevention simultaniously prevents anxiety, fear and phobia which are often induced by discomfort and pain encountered during dental treatment. Nowadays it is almost always possible to provide painless dental treatment because there are a variety of highly potent and low toxic anesthetics. Nevertheless, a local anaesthetic injection is considered to be the main cause of pain and the only way to prevent the pain caused by dental treatment at the same time. In the mid 1990s, a concept of computer-controlled local anesthetic delivery (C-CLAD) was developed in order to lessen the patient’s discomfort associated with injections. C-CLAD systems allow the use of standard, supplemental or modified injection techniques while using special designed computer systems with the purpose of minimizing human mistakes during the application of anesthesia. Therfore, C-CLAD is convenient for children since pain, fear and anxiety are reduced in comparison to use of standard manual syringes. The first C-CLAD system introduced to the market was The Wand (Milestone Scientific, Livingston, NJ, SAD). Subsequent versions from the same manufacturer were named Wand Plus, CompuDent and STA (Single-Tooth-Anesthesia). The market offers a few C-CLADs today. What they all have in common is the option to control speed, flow and pressure while applying local anesthetics, as well as stress reduction for both patients and operators. Still, these systems are expensive and they require additional time and equipment as well as the operator’s knowledge of the device’s handling.
