Abstract

Veza izmeĎu oralnog i sistemskog zdravlja već je dugo predmet istraţivanja. U
brojnim studijamaje dokazano kako je osnovna parodontna bolest povezana s
povećanim rizikom za bolesti kardiovaskularnoga sustava. Oralne infekcije i
koronarna oboljenja su bolesti koje imaju zajedničku etiologiju u nekoliko dodirnih
točaka. Najčešći oblik parodontne bolesti jesu kronični i agresivni parodontitis.
Javlja se uglavnom kod starijih osoba, ali moţe se javiti i kod djece te adolescenata.
Danas je poznato da je parodontna bolest primarno uzrokovana mikroorganizmima.
Upalna reakcija oralnoga vezivnog tkiva koja je izazvana mikroorganizmima za
posljedicu ima povećanu popustljivost endotela koji dopušta migraciju bakterija u
perivaskularni prostor. Veza izmeĎu lošega oralnog zdravlja i kardiovaskularnih
bolesti ogleda se u tome što oralne infekcije mogu izazvati upalu koja moţe imati
posljedice na kardiovaskularne bolesti. Moţe se danas sa sigurnošću reći da postoji
veza parodontnih bolesti i odlaganja masnih naslaga na arterijske zidove što dovodi
do pojave krvnih ugrušaka koji izazivaju aterosklerotsku bolest krvnih ţila. Postoji
mišljenje da bakterije kod parodontne bolesti ulaze u krvotok preko prostora oko
zuba, bivaju inkorporirane u arterijski plak te pogoršavaju upalu. Pored
mikroorganizama treba spomenuti i ostale zajedničke čimbenike rizika kao što su
dob, spol, pušenje, konzumiranje alkohola i socioekonomski status, ali ne treba
zaboraviti ni druge moguće čimbenike tih bolesti.

Abstract (english)

The relationship between oral and systemic health has been investigated for a long
time. Numerous studies have proved that the periodontal disease is associated with
an increased cardiovascular risk. Oral infections and coronary heart disease share
their etiology in several aspects. The common two forms of periodontal diseases are
chronic and aggressive periodontitis. It occurs mainly in older population, but can
also appear in children and adolescents. It is now known that periodontal disease is
caused by microorganisms. The inflammatory reaction of oral connective tissue
caused by microorganisms results in increased endothelial permeability that allows
the migration of bacteria in the perivascular space. The association of poor oral
health with cardiovascular disease is reflected in the fact that oral infections can
cause inflammation, which may have implications for cardiovascular disease. Today,
it can be assumed with certainty that there is a link between periodontal disease and
disposal of fatty deposits on arterial walls leading to the occurrence of blood clots
that cause atherosclerotic vascular disease. It is considered that in periodontal disease
bacteria enter the circulation through the area around the teeth; they are incorporated
into the arterial plaques and they worsen the inflammation. In addition to
microorganisms, shared risks such as age, gender, smoking, alcohol consumption and
socioeconomic status should be mentioned, but we should not forget other possible
risk factors of these diseases too.
