Abstract

Augmentacija maksile ekstraoralnim koštanim transplantatom složen je kirurški zahvat, no
često neophodan kod velikih koštanih deficita. Najčešće ekstraoralno donorsko mjesto je
greben kriste ilijake, prvenstveno zbog laganog pristupa, niskog stupnja komplikacija te
količine kortikalne i spongiozne kosti koje nudi ovo donorsko mjesto. Bez obzira na velik
tehnološki napredak u zamjeni koštanih supstanca umjetnim materijalima, svježa autologna
kost smatra se zlatnim standardom u augmentativnim postupcima. Ovo nam donorsko mjesto
služi kao rezervoar velike količine kosti kod velikih koštanih defekata.
U ovom su radu opisane tehnike uzimanja autologne kosti, indikacije za upotrebu ekstraoralnog
donorskog mjesta, prednosti, ali i nedostatci ovog augmentativnog postupka. Uz kirurške
tehnike navedene su i moguće komplikacije te stopa preživljavanja i uspješnosti integracije
koštanog nadomjestka na primajuću regiju te oseointegracija implantata u novostvoreni koštani
nadomjestak. Također, dan je kratak osvrt na anatomske značajke i dostupnost biomaterijala
koji mogu pomoći ne bi li se ostvario optimalan rezultat.

Abstract (english)

Maxilla augmentation with extraoral bone graft is a complex but often necessary surgical
procedure for major bone deficits. The most common extraoral donor site is iliac crest, primarily
due to easy access, low complication rate and the cortical and cancellous bone volume this
donor site has to offer.
Regardless of the technological advances in the use of artificial materials for bone replacements,
fresh autologous bone remains the golden standard for augmentation procedures. This donor
site serves as a large bone reservoir for major bone defects.
This paper describes the autologous bone harvesting techniques, indications for the use of
extraoral donor site as well as advantages and disadvantages of this type of augmentation
procedure. It examines surgical techniques, potential complications as well as survival and
success rates of bone graft integration into the recipient area and osseointegration of the implant
into the newly formed bone tissue. It also contains a brief overview of anatomical features and
available biomaterials that can be used to achieve optimal results.
