Abstract

Svrha ovog rada je kritički pregled farmakoterapijskog liječenja temporomandibularnih poremećaja temeljem dostupne znanstvene literature. Pristup u rješavanju boli ovisi o pažljivoj dijagnozi i multifaktorijalnoj etiologiji. Cilj terapije je umanjiti bol zglobova, mišića i pridruženu upalu. Pri tome prednost imaju reverzibilni, konzervativni postupci, a farmakoterapija ih često nadopunjuje. Danas, farmakoterapija u kombinaciji s fizikalnom terapijom i terapijom upravljanja stresom predstavlja vrlo učinkovit način liječenja temporomandibularnih poremećaja, ali nužan je oprez uslijed čestih nuspojava.
Ovim radom obuhvaćeno je 15 istraživanja, objavljenih od 1999. do danas. Uključena istraživanja ispitivala su terapijski učinak nesteroidnih antireumatika (NSAR), antidepresiva, anksiolitika, antiepileptika, lokalnih anestetika, natrij hijaluronata te botulin toksina. Premalo je znanstvenih dokaza u korist ijedne terapijske skupine. Tek usporedbom farmakoterapije i placeba dokazana je njena učinkovitost. Takvi rezultati posljedica su varijabilne metodologije uključenih istraživanja, malog broja ispitanika po skupinama i kratkog perioda praćenja. Radom je ipak prikazana korisnost farmakoterapije u liječenju TMP-a no nužne su daljnje dobro vođene RCT studije koje će odrediti koja je farmakološka intervencija najbolja.

Abstract (english)

The aim of this sistematic review is to provide a critical review of the pharmacoterapy of temporomandibular disorders based on presently available data from scientific literature. Pain disolving approach depends on careful diagnosis and multifactorial etiology. The goal of TMD management is to decrease joint pain, muscle pain and associated inflammation. Non-operative methods take advantage, and pharmacotherapy often complements them. Today, together with physical therapy and stress management pharmacotherapy stands out as very efficient way to disolve TMD but often side effects require caution.
This review includes 15 studies, published from 1999s till today. Included studies reviewed therapeutic effect of nonsteroidal antiinflammatory drugs (NSAIDs), antidepressants, anxiolytics, antiepileptics, local anesthetics, sodium hyaluronate, botulinum toxin. There is lack of scientific evidence in favour of any therapeutic group. Only comparing pharmacotherapy and placebo demonstrated its effectiveness.Those results are due to variable methodology of included studies, small sample size and short duration of follow-up. However, this review confirmed the use of pharmacotherapy in TMD but more well designed RCT studies are needed to determine which pharmacologic intervention is the most effective.
