Abstract

Pojam vođene koštane regeneracije datira unatrag tridesetak godina. Tijekom razvoja tehnologije i materijala, razvila se cijela lepeza dostupnih membrana za upotrebu. Bez obzira na materijal njegova je uloga do danas ostala ista: sprječavanje proliferacije mekih tkiva koja posljedično omogućava koštanu regeneraciju. Međutim, sve veći naglasak stavlja se na aktivnu ulogu membrane, s obzirom na to da je dokazana prisutnost molekula koje direktno utječu na koštanu regeneraciju na površini membrane. Iako do danas nije pronađen konsenzus akademske zajednice po pitanju veličine pora membrane i njezinog utjecaja na sam postupak, većina znanstvenika smatra kako membrana treba imati određenu poroznost kako bi se omogućila angiogeneza područja, a bez koje nema koštane regeneracije. Potrebno je spomenuti augmentativne koštane materijale, koji su nužni kod većih augmentativnih postupaka. Oni pokazuju osteokonduktivna, a pojedini i osteoinduktivna svojstva. Iako je ostvaren značajan napredak u tehnološkom dosegu, zlatni standardi koji su određeni 90-ih godina prošlog stoljeća perzistiraju i danas. Ekspandirana politetrafluoroetilenska membrana i autologna kost predstavljaju materijale s kojima se svi današnji materijali uspoređuju i najviši su standard kod vođene koštane regeneracije. Iako danas vođenu koštanu regeneraciju smatramo predvidivim i rutinskim zahvatom, u slučajevima većih horizontalnih i vertikalnih augmentacija iziskuje višu stopu kiruškog znanja i iskustva te sa sobom nosi i višu stopu mogućih komplikacija. Po pojedinim istraživanjima taj postotak iznosi i do 50%. Pažljivim odabirom materijala, poznavanje svojstava membrana i augmentativnih materijala, kao i pridržavanje kirurškog protokola uz pacijentovu suradnju omogućuje uspješno provođenje velikih i zahtjevnih postupaka vođene koštane regeneracije.

Abstract (english)

The term “guided bone regeneration” has existed and has been used for the last thirty years. During this period, many different membranes have been invented and patented. Still, the role of the membrane is the same: not allowing the proliferation of soft tissue, thus allowing bone regeneration to happen. However, in the last decade scientists discovered that the membrane does not have only a passive role as it had been thought earlier. Today, there are proofs of active role of membranes and it is the nowadays` subject of recent scientific papers. Nevertheless, there is still no consensus about the ideal pore size in membranes. Most scientific papers suggest that pores are important because they allow proper angiogenesis and oxygenation to the place of defect. Most of the augmentative materials show osteo-conductive properties and some of them have osteo-inductive properties which are important for proper bone regeneration. Autogenous bone and e- PTFE membranes remain the so-called golden standards as it was set in 1990s. Although there are many benefits of GBR, it is not a flawless procedure, especially in cases of large bone augmentations (vertical and horizontal defects). Some authors suggest that percentage of complications in that case are up to 50%. This shows that guided bone regeneration procedure must be taken seriously, with proper surgical and theoretical knowledge and then the percentage of complications can be decreased to an acceptable one.
