Abstract

Anomalije klase III ortodontske su anomalije kod kojih se donji prvi kutnjak nalazi mezijalnije nego gornji pa meziobukalna kvržica gornjeg prvog kutnjaka okludira distalnije. Uzroci takvom odnosu mogu biti dentoalveolarne i skeletne anomalije. Skeletne anomalije obuhvaćaju maksilarni retrognatizam, mandibularni prognatizam ili njihovu kombinaciju. Incidencija anomalija klase III u bijeloj rasi značajno je manja negoli u azijskoj populaciji te iznosi oko 1 %, dok u Azijata doseže i do 14 %. Anomalije klase III razvojno su stanje. One, najčešće, nisu uzrokovane nekim patološkim procesom, već odmakom od normalnog razvoja do kojeg je došlo interakcijom više faktora koji utječu na rast i razvoj. Faktori mogu biti genetski i okolišni. Dijagnoza se postavlja na temelju laterolateralnog kefalograma, odnosno RTG kefalometrije. Anomalije klase III jedne su od najizazovnijih ortodontskih problema. Metode liječenja klase III uključuju modifikaciju rasta dok potencijal rasta i razvoja još postoji, kamuflažnu terapiju te kombinaciju ortodontske terapije sa zahvatima ortognatske kirurgije jednom kada su rast i razvoj završeni.

Abstract (english)

Class III malocclusion is orthodontic anomaly in which the lower first molar is located more mesially, so the mesiobuccal cusp of the upper first molar occludes more distally. Dentoalveolar and skeletal anomalies can be the cause of such relationship. Skeletal anomalies include maxillary retrognathism, mandibular prognathism, or their combination. With approximate incidence of about 1% the incidence of class III malocclusion in the white race is significantly lower than in the Asian population where it reaches up to 14%. Class III malocclusion is a developmental condition. Most often, it's not caused by a pathological process, but by a departure from normal development that occurs through the interaction of several factors affecting growth and development. Factors can be genetic and environmental. The diagnosis is made based on a laterolateral cephalogram, or X-ray cephalometry. Class III malocclusion is one of the most challenging orthodontic problems. Class III treatment methods include growth modification while the potential for growth and development still exists, camouflage therapy and combination of orthodontic treatment with orthognathic surgery once growth and development have ceased.
