Abstract

Za uspješno provođenje fiksnoprotetske terapije potrebna je suradnja sdrugim granama dentalne medicine. Takvim multidisciplinarnim pristupom pacijent će biti kvalitetno i cjelovito saniran. Pri planiranju rada pacijenta treba upoznati s konačnim ishodom terapije i na taj način izbjeći nerealna očekivanja. Brušenje uporišnih zuba ovisi o više čimbenika, o vrsti nadomjeska, položaju zuba u zubnom luku, broju preostalih zuba te materijalu kojim se zub nadomješta,. Uvijek je potrebno brusiti stepenicu (zaobljenu, rjeđe pravokutnu) osim u izuzetnim slučajevima (ranije brušenje, izrazito gracilni donji incizivi i jako nagnutih i konveksnih molara). Pri brušenju je osobito važno pratiti morfologiju zuba. Opseg brušenja zuba potrebno je prilagoditi vrsti budućeg nadomjeska i gradivnom materijalu (za potpuno kovinske krunice potrebno je znatno manje prostora nego za potpunu keramiku). Najčešće pogreške koje se događaju pri brušenju vezane su uz preobilno ili nedovoljno brušenje, nepravilno brušenje stepenice, nepravilne rubove, nepolirane plohe, preveliko zakošavanje stjenki zuba pri čemu se ne prati osnovna morfologija zuba i narušava retencija i rezistencija.

Abstract (english)

For the successful implementation of a fixed prosthodontic therapy it is necessary to include other branches of dental medical as well. With such a multidisciplinary approach a patient will be completely and quality taken care of. It is necessary to introduce the patient with the final outcome of the therapy when planning the work thus avoiding unreal expectations. Tooth preparation depends on many factors; the type of restoration, the position of a tooth in an arc, the number of remaining teeth and material which is used as tooth substitute. In such cases it is needed to grind champfer (rarely shoulder) cervical finish line except in rare cases, such as; earlier preparation, extremely thiny lower incisors and lean and convex molars. During preparation it is extremely important to follow tooth morphology. The dimeter of grinding should be adjusted to the type of future restoration and constructive material (for complete metal crowns there is a considerably less need for the space than for the all ceramics). The most frequent failures during grinding are caused by the inappropriate amount of grinding, inappropriate preparation of cervical finish line,irregular edges, unpolished plane, tilting tooth plane which is not in line with the tooth morphology thus distorts retention and resistance.
