Abstract | Maksila i mandibula bogato su opskrbljene krvnim žilama i živcima. Kod postojanja određene patologije zuba, uz detaljno uzetu anamnezu, obavljen klinički pregled i analiziranu rendgensku snimku za što precizniju dijagnozu i bolji plan liječenja koristimo se dodatnim dijagnostičkim testovima. Testovi senzibiliteta često se krivo zamjenjuju s testovima vitaliteta. Testovi senzibiliteta ispituju inervaciju zubne pulpe, a testovi vitaliteta ispituju njegovu vaskularizaciju. Negativan rezultat na test senzibiliteta znači da je zub denerviran, a to ne znači nužno da je zub i avitalan. Vitalitet ovisi o vaskularizaciji zuba koja može biti očuvana iako je zub denerviran te ga ispitujemo testovima vitaliteta. U ovom radu pobliže je opisana metoda mjerenja vitaliteta laser Doppler flowmetrom (LDF). LDF se koristi za ispitivanje vitaliteta kože, oralne sluznice, gingive, alveolarne kosti, nepčane sluznice i pulpe
zuba. Razlikom frekvencija odlaznih i reflektiranih zraka možemo mnogo saznati o vitalitetu mjerenog područja što nam uvelike pomaže u razumijevanju patologije s kojom se susrećemo. Unatoč brojnim istraživanjima i obećavajućim mogućnostima, nije pronašao širu kliničku primjenu zbog svoje visoke cijene, potrebne dodatne edukacije, ometanja signala okolnim tkivom, potrebe za što boljom izolacijom ispitivanog područja i što manjom pomičnošću pacijenta i uređaja, učestalog kalibriranja te opširnih restauracija koje onemogućavaju mjerenje. |
Abstract (english) | The maxilla and the mandible are richly supplied with blood vessels and nerves. In the presence of a certain dental pathology, with a detailed anamnesis, performed clinical examination and analyzed X-ray for the most accurate diagnosis and better treatment plan, we use additional diagnostic tests. Sensitivity tests are often misunderstood with vitality tests.
Sensitivity tests examine the innervation of the dental pulp, and sensitivity tests examine its vascularization. A negative result on the sensitivity test means that the tooth is denervated, which does not necessarily mean that the tooth is avital. Vitality depends on its vascularization, which can be preserved even if the tooth is denervated, and it is examined by
vitality tests. In this paper, the method of measuring the vitality of a laser Doppler flowmeter (LDF) is described in more detail. LDF is used to examine the vitality of the skin, oral mucosa, gingiva, alveolar bone, palatine mucosa and tooth pulp. By the difference between the frequencies of the outgoing and reflected rays, we can learn a lot about the vitality of the measured area, which greatly helps us to understand the pathology we encounter. Despite numerous researches and promising possibilities, it did not find wider clinical application due to its high cost, necessary additional education, signal interference with the surrounding tissue, need for better isolation of the examined area and less patient and device mobility, frequent calibration and extensive restorations that make measurement impossible. |
