Abstract

Kao posljedica novih znanstvenih i kliničkih spoznaja te dugogodišnjeg iskustva i brze izmjene znanja, 2017. godine u Chicagu, na inicijativu Američke akademije za parodontologiju i Europske federacije za parodontologiju, održana je konferencija s ciljem obnove Klasifikacije parodontnih bolesti i stanja iz 1999. godine. Prvi konsenzus donosi definiciju i dijagnostičke kriterije za parodontno i gingivno zdravlje te za upalu na parodontu koji može biti normalan ili reduciran. Pri postavljanju dijagnoze gingivne
upale glavni parametar trebalo bi biti krvarenje pri sondiranju. Prihvaćeno je da se kod pacijenta s upalom gingive može postići potpuno izlječenje, dok pacijent s parodontitisom ostaje pacijent doživotno. Reorganiziran je širok spektar gingivnih bolesti i stanja koja nisu uzrokovana plakom. Drugi konsenzus prihvaća promjene vezane za parodontitis. Dogovoreno je da prema
trenutnim patofiziološkim spoznajama možemo razlikovati tri oblika parodontitisa: nekrotizirajući parodontitis, parodontitis kao manifestacija sistemske bolesti i parodontitis koji ujedinjuje ranije oblike bolesti „agresivni“ i „kronični“ u jednu skupinu. Parodontitis se dodatno karakterizira prema stadijima i razredima. Trećim konsenzusom predstavljene su sistemske bolesti povezane s gubitkom potpornih parodontnih tkiva. Donesena je i nova klasifikacija gingivnih recesija vezana uz terapiju koja se bazira na aproksimalnom gubitku kliničkog pričvrstka. Dan je osvrt na utjecaj okluzalnih sila i protetskih faktora na parodont.
Četvrti konsenzus predstavlja klasifikaciju periimplantatnih bolesti i stanja gdje su jasno definirane karakteristike periimplantatnog zdravlja, periimplantatnog mukozitisa i periimplantitisa.

Abstract (english)

As a result of new scientific and clinical breakthroughs and the rapid exchange of knowledge in 2017 a conference was held with the aim of updating the 1999 Classification of Periodontal Diseases and Conditions. The first consensus provides diagnostic criteria for periodontal and gingival health and periodontal inflammation in periodontium that can be normal or reduced. It was agreed that bleeding on probing should be the primary parameter for diagnosing gingivitis. It is accepted
that a patient with gingivitis can revert to a state of health while a patient with periodontitis remains a patient for life. The second consensus accepts changes related to periodontitis. It was agreed that according to current knowledge of pathophysiology three forms of periodontitis can be identified: necrotizing periodontitis, periodontitis as a manifestation of systemic disease and periodontitis that unites earlier forms of disease "aggressive" and "chronic" into one group.
Periodontitis is further characterized on the basis of a staging and grading system. The third consensus presents systemic diseases associated with the loss of supporting periodontal tissues. A new classification of gingival recessions related to therapy based on proximal loss of clinical attachment has also been adopted. A review of the influence of occlusal forces and prosthetic factors on the periodontium has been given. The fourth consensus presents the characteristics of peri-implant health, peri-implant mucositis, and peri-implantitis.
