Abstract

Morfeja je podtip lokalizirane skleroderme i rijetko je stanje karakterizirano prekomjernim odlaganjem kolagena, bez ili s minimalnim otvrdnućem zahvaćene lezije koja može biti hipopigmentirana ili hiperpigmentirana. Dok skoro u svim slučajevima sklerozirajući proces isključivo zahvaća kožu, opisani su rijetki slučajevi u kojima je uključena i sluznica. Prikazan je rijetki slučaj morfeje na usnama, odgovarajućoj sluznici usna i gingive u okolini zuba 22 kod tridesetosmogodišnje pacijentice. Pacijentica je povremeno bolovala od manjih želučanih tegoba, no inače je bila zdrava. Morfeja je bila uzrokovana traumom. Naveden je popis diferencijalnih dijagnoza. Kod dijagnosticiranja bijelih lezija u usnoj šupljini i na koži lica najveću važnost ima histopatološka analiza. U ovom slučaju morfeja je bila uspješno liječena pomoću intralezijskih injekcija kortikosteroida tijekom osam tjedana te uporabom topikalne betametazon kreme tri puta na dan.

Abstract (english)

Morphea is subtype of localized scleroderma and is a rare condition characterized by an excess collagen deposition and with minimal or no induration of affected lesions, which can be either hypopigmented or hyperpigmented. While the sclerosing process exclusively affects the skin in almost all cases, anecdotal cases have been described with associated mucosal involvement. In this paper is presented a rare case of mophea on the lips, as well as corresponding labial mucosa and gingiva adjacent to the tooth 22 in a female patient aged 38. The patient suffered from occasional minor gastric disturbances, and was otherwise healthy. Morphea was preceeded by trauma. A list of differential diagnosis is included here. When diagnosing oral and facial white lesions, histopathology is of utmost importance. In this case, morphea was succesfully treated with eight weekly intralesional corticosteroid injections in combination with topical bethametasone ointment, which was applied three times a day.
