Abstract

Implantoprotetska terapija zahtijeva detaljno planiranje kako bi se postigla optimalna funkcija,
estetika i prirodnost budućeg nadomjestka. Svaka terapija započinje anamnezom i kliničkim
pregledom s analizom fotografija, CBCT-a (Cone Beam Computed Tomography) ili
ortopantomografske snimke. Slijedi atraumatska ekstrakcija i pozicioniranje implantata u točno
određenoj orofacijalnoj, meziodistalnoj i apikokoronalnoj dimenziji gdje je ključno sačuvati
dovoljnu debljinu bukalne stjenke radi postizanja optimalnog nivoa periimplantatne mukoze.
Osim pozicije implantata, na konačnu će estetiku utjecati odabir gradivnog materijala
implantata, spoj implantata i nadogradnje, spoj nadogradnje i protetske suprastrukture te
obradivost površine vrata implantata. Zatim slijedi oblikovanje mekih tkiva gingiva formerom
ili privremenim nadomjescima. Bitno je naglasiti da je za predvidljivost izgleda mekih tkiva
najvažniji prognostički čimbenik gingivalni fenotip pacijenta. Izrada privremenoga protetskog
rada pacijentu osigurava funkcijsku, fonetsku i estetsku nadoknadu, a terapeutu će služiti kao
alat za oblikovanje marginalne granice gingive te očuvanje i oblikovanje papile. Kako bi se
udovoljilo zahtjevima „bijele” estetike, pozornost se obraća na prirodan izgled budućeg
nadomjestka koji se najbolje može ostvariti upotrebom keramičkih materijala.

Abstract (english)

Dental implant therapy requires detailed planning in order to achieve optimal function,
aesthetics and a more natural look of future dental prosthetics. Therapy begins with a medical
history, clinical examination with analysis of photographs and CBCT (Cone Beam Computed
Tomography) or panoramic radiographs. It is followed by atraumatic extraction and precisely
defined implant placement in orofacial, mesiodistal and apicocoronal dimension. In order to
achieve optimal soft tissue level it is crucial to preserve sufficient thickness of the buccal bone
plate. Except implant position, the final aesthetic results depend on implant material, the
implant-abutment connection, retention system and the design of the neck of the implant. Soft
tissue conditioning is achieved with healing abutments or provisional restaurations. The most
important factor influencing the final result of soft tissue aesthetic is gingival biotype.
Provisional restorations should be used to evaluate aesthetic, phonetic and occlusal function
prior the delivery of the final implant restoration, while preserving and/or enhancing the
condition of the peri-implant tissues. In order to meet the requirements of "white" aesthetics,
great attention is paid to the natural appearance of future dental prosthetics, which can best be
achieved by using ceramic materials.
