Abstract

Jednokomadni makrodizajn građe implantata terapijski je oblik u dentalnoj implantologiji, kod kojeg je protetski opteretiva nadogradnja sastavni dio i fizički neodvojiva komponenta od unutarkoštanog implantatnog tijela. Osim titanija/titanijskih legura, u suvremenoj dentalnoj medicini za izradu monolitnih implantata sve češće se razvijaju i bezmetalni materijali poput itrijem ojačane cirkonij-oksidne keramike, koja osim poboljšanih mehaničkih pokazuje i obećavajuća oseointegracijska svojstva. Svrha rada je na znanstveno utemeljenim činjenicama prikazati mogućnost, efikasnost i sigurnost primjene jednokomadnih dentalnih implantata u implantoprotetskom zbrinjavanju, neovisno o njihovom gradivnom materijalu. Cilj je prikazati imedijatni ili odgođeni kirurški/protetski protokol kod sustava različitih proizvođača i njihov utjecaj na terapijske ishode te izraditi meta-analizu rezultata pretrage i pregleda stručne i znanstvene literature. Pretraživanje putem PubMed internet stranice rezultiralo je pronalaskom 413 članaka od kojih je prema navedenim kriterijima uključivanja/isključivanja njih 13 bilo dio meta-analize. Praćeni su ishodi 595 ugrađenih jednokomadnih dentalnih implantata promjera ≥ 3 mm, različitih duljina, izgubljeno ih je 19, što ukupno čini 96,80% preživljenja jednokomadnih dentalnih implantata neovisno o materijalu iz kojih su izrađeni. Praćeni jednokomadni implantatni sustavi pokazali su visoke stope uspješnosti kod potpore pojedinačnim krunicama i mostovima kratkih raspona te se ovi pozitivni rezultati tek trebaju potvrditi kvalitetnim dugoročnim studijama na većem uzorku ispitanika.

Abstract (english)

A one-piece macro design of dental implants is a therapeutic form, in which a prosthetically loadable superstructure is an integral part and physically inseparable component of the intra-bone implant body. Apart from titanium and titanium alloys, development of non-metallic materials such as yttrium stabilized zirconia is increasingly fostered for fabrication of monolithic implants in modern dental medicine, which, in addition to improved mechanical properties, show also promising osseointegration properties. The purpose of this study was to show the possibility, efficiency, and safety of using one-piece dental implants in implant-prosthetic therapy, based on scientific facts and regardless of the material they are made of. The aim was to present an immediate or delayed surgical/prosthetic protocol for implant systems of different manufacturers and their impact on therapeutic outcomes, as well as to perform a meta-analysis of the study results and review of the professional and scientific literature. In the search through PubMed 413 articles were found, 13 of which became part of the meta-analysis following the prescribed inclusion/exclusion criteria. The outcomes of 595 inserted one-piece dental implants with a diameter of ≥ 3 mm and different lengths were analyzed. Nineteen implants were lost, accounting for the survival rate of 96,80% for one-piece dental implants regardless of the material they are made of. The follow-up data showed high success rates of one-piece implant systems in support of single crowns and short span bridges. These positive results are still to be confirmed by qualitative long-term studies with a larger number of subjects.
