Abstract

Jedan od glavnih preduvjeta za postizanje i održavanje uspješne oseointegracije jest prisutnost dostatnog volumena zdrave kosti na mjestu ugradnje, što ne podrazumijeva samo dovoljnu visinu kosti, koja će omogućiti ugradnju implantata odgovarajuće dužine, već i dovoljnu širinu alveolarnoga grebena.
Kirurške tehnike za koštanu augmentaciju alveolarnoga grebena, koje se najčešće primjenjuju u modernoj dentalnoj medicini zahvaljujući predvidivim rezultatima, sigurnosti postupka, značajnom postotku uspješnosti te relativno niskoj stopi postoperativnih komplikacija, uključuju vođenu regeneraciju kosti, širenje kosti te primjenu koštanog blok-transplantata.
Važan iskorak u obnovi koštanih tkiva učinjen je uvođenjem barijernih membrana čija je svrha osigurati nesmetane uvjete za nastanak i formiranje nove kosti, bez mogućnosti inkorporacije mekih tkiva u taj ograničeni prostor. Titanska mrežica pokazala se, zahvaljujući svojim prednostima, u kombinaciji s raznim nadomjesnim materijalima kod brojnih augmentativnih postupaka iznimno uspješnom.
Iako još uvijek ne postoji idealan transplantatni materijal koji će u potpunosti ispuniti sve kriterije i osigurati savršene rezultate prilikom upotrebe u koštanoj augmentaciji, najbliže je tomu, sa svojim osteogenim, osteoinduktivnim, osteokonduktivnim svojstvima, vrlo često upotrebljavana autogena kost.

Abstract (english)

One of the most important prerequisites for acheiving and maintainig successful osseointegration is the presence of a sufficient volume of healthy bone at recipient site, which includes not only bone of sufficient height to allow the insertion of an implant of appropriate length, but also a sufficient alveolar ridge width.
Surgical techniques for alveolar ridge augmentation which are most performed in modern dentistry, owing predictable results, process safety, high successful rate and relatively low postoperative complications, includes guided bone regeneration, ridge-split technique and block graft augmentation.
An immportant step forward in the bone regenration has made by introducing barrier membranes whose purpose is to ensure secure conditions for generating and forming new bone, without possibility of soft tissue ingrowth in this restricted area. Titanium mesh, due to unique benefits in combination with various grafting materials has shown in numerous augmentative procedures its raliability and utility.
Althought the ideal graft material which would insure perfect bone grafting results has no available yet, close to this is often used autogenuous bone with its osteogenic, osteoinductive and osteoconductive capacitys.
