Abstract

Svrha ovog rada je prikazati remineralizaciju i infiltracijsku tehniku kao minimalno invazivnu metodu terapije oštećenja cakline. Bijele mrlje jedan su od čestih nalaza u trajnoj denticiji a prikazane metode omogućavaju poštedu tvrdih zubnih tkiva i na taj način odgađaju protetska i invazivnija rješenja za kasnije. Prevalencija razvojnih defekata cakline trajnih zubi u razvijenim zemljama kreće se u rasponu od 9 do 68 % podjednako kod muškaraca i žena. Klinička slika i izgled defekta ovisi o stadiju razvoja zuba u kojem dolazi do štetnog djelovanja lokalnog ili sistemskog faktora. S druge strane, dugotrajno djelovanje kiselih metaboličkih produkata dovodi do površinskog omekšavanja cakline, zatim bijele mrlje te na kraju do uznapredovale caklinske karijesne lezije. U prvom prikazu slučaja infiltracijskom smolom ICON tretirane su bijele lezije nekarijesnog podrijetla, a u drugom difuzne bijele promijene na vestibularnim plohama za koje pretpostavljamo da su nastale kao posterupcijska oštećenja. Infiltracijska metoda omogućava popunjavanje mikroporoznosti hipomineralizirane cakline što omogućava maskiranje caklinskog defekta i jača strukturu cakline, a u slučaju kada postoji karijesna lezija zatvara navedene pore i sprečava napredovanje karijesne lezije.

Abstract (english)

Enamel is the toughest tissue in the human body. It protects dentin and pulp and ensures tooth strength due to its high mineral content. The purpose of this paper is to present remineralization and infiltration techniques as a minimally invasive method of treatment of enamel damage. White spots are one of the common findings in permanent dentition, and the methods presented allow the tough dental tissue to be spared and thus postpone prosthetic and invasive solutions for later. The prevalence of developmental defects of enamel in the permanent dentition in developed countries ranges from 9 to 68% in both men and women. The clinical picture and appearance of the defect depends on the stage of the tooth development at which the local or systemic factor is detrimental. On the other hand, the long-term action of acidic metabolic products leads to the surface softening of the enamel, then the white spots, and finally to the advanced caries lesion on the enamel. The first case report is aimed at reporting the treatment of ICON infiltration resin on white lesions of noncaries origin, and the second, diffuse white lesions on vestibular surfaces that were assumed to have occurred as post eruption lesions. The infiltration method allows the microporosity of the hypomineralized enamel to be filled, which enables masking of the enamel defect and strengthens the structure of the enamel, and in the case of a caries lesion closes the pores and prevents the caries lesion from progressing.
