Abstract

Hematološki bolesnici lijeĉeni antiresorptivnom terapijom predstavljaju rizik za nastanakmedikamentozne osteonekroze ĉeljusti.Incidencija pojavljivanja osteonekroze ĉeljusti uzrokovane lijekom nije visoka, ali je tijekom zadnjih godina u porastu. To se povezuje sa sve većim brojem bolesnikalijeĉenih antiresorptivnom i antiangiogenom terapijom. Takvi bolesnici zahtjevaju posebnu stomatološku skrb. Ĉesto je rijeĉ o teško oboljelim osobama, a osteonekroza ĉeljusti još dodatno narušavanjihovu kvalitetu ţivota.Osteonekroza ĉeljusti manifestira se izloţenom kosti koja ne cijeli duţe od osam tjedana i varira svojom veliĉinom i simptomima. Lijeĉenje ovisi o mnogim ĉimbenicima koji ukljuĉujudob, spol, stanje bolesnika, stupanj bolesti i veliĉinu lezije. Pristup lijeĉenju je individualan.Razlikujemo konzervativno lijeĉenje, kirurško lijeĉenje i pomoćno nekirurško lijeĉenje.Izrazito je bitna dobra suradnja hematologa koji propisuje terapiju i doktora dentalne medicine u svrhu što bolje prevencije osteonekroze ĉeljusti.Prije uvoĊenja terapije, preporuka je obaviti stomatološki pregled i sve zahvate za sanaciju usne šupljine.Ako postoji potreba za invazivnim stomatološkim zahvatom kod visoko riziĉnih pacijenata, doktor dentalne medicine trebao bi konzultirati hematologa o eventualnom prekidu terapije.U svrhu što bolje prevencije, rane dijagnoze i kvalitetnog lijeĉenja, potrebna je dobra suradnja s hematologom i što bolja edukacija doktora dentalne medicine.

Abstract (english)

Hematology patients receiving antiresorptive therapy represent a group of patients in risk of developing a medication-related osteonecrosis of the jaw. The incidence of a medication-related osteonecrosis of the jaw is not high, but it has increased over the last few years. This increase can be associated with a growing number of patients on antiresorptive and antiangiogenic therapy. Those patients require special dental care. They are often severely ill to begin with, and osteonecrosis of the jaw deteriorates their quality of life even more. Osteonecrosis of the jaw manifests itself with an exposed bone that does not heal for more than eight weeks and it varies in size and symptoms. The treatment depends on many factors including age, gender, the patient’s condition, the stage of the illness and the size of the lesion. The approach to the treatment is individual. There are three different types of treatments available: the conservative treatment, the surgical treatment and the adjuvant non-surgical treatment. In order to prevent osteonecrosis of the jaw in a better way, it is essential to improve the quality of the cooperation between the doctor of dental medicine and the hematologist involved. Prior to starting the therapy, it is recommended for a patient to have a dental examination done and to finish all necessary dental procedures. If there is a need for an invasive dental surgery in high-risk patients, a doctor of dental medicine should consult with a hematologist about a possible discontinuation of the therapy. To achieve better prevention, an early diagnosis and a quality treatment, it is extremely important to have a good cooperation with the hematologist and to further educate doctors of dental medicine.
