Abstract

Rječnik protetskih pojmova definira bruksizam kao parafunkcijsko škripanje ili stiskanje zubima ili oralnu naviku koja se sastoji od nekontroliranog ritmičnog ili nejednoličnog nefunkcijskog brušenja, škripanja i stiskanja zubima pokretima mandibule koji mogu dovesti do okluzalne traume.
Iako predstavlja čest problem u populaciji, etiologija mu još nije u potpunosti razjašnjena. Poseban problem predstavlja istraživanje bruksizma kod djece. Ograničavajući faktor kliničke su studije u kojima su ispitanici maloljetnici.
Faktori koji se najčešće povezuju s bruksizmom višestruki su i u njih ubrajamo osobnost, ponajprije anksioznost i crte ličnosti koje ukazuju na napetost djeteta, te stres, kao i periferne (morfološke) čimbenike (okluzija i orofacijalna anatomija).
Metode koje se koriste u dijagnozi bruksizma kod djece zbog toga moraju biti multifaktorijalne te uključivati upitnike i intervjue s djecom i njihovim roditeljima, klinički pregled (intraoralni i ekstraoralni), a u nekim slučajevima EMG te polisomnografiju tijekom spavanja.
Posljedice bruksizma možemo podijeliti na dentalne i ostale. U dentalne ubrajamo lokaliziranu ili generaliziranu istrošenost zuba, nekrozu ili frakturu zuba, upale gingive i parodonta, a neki čak navode i prijevremenu erupciju trajnih nasljednika. U ostale posljedice noćnog bruksizma ubrajamo hipertrofiju žvačnih mišića, glavobolju, bol pri žvakanju i disfunkciju TMZ-a.
U terapiji noćnog bruksizma kod djece dostupne su nam intraoralne naprave, tj. udlage, psihoterapija te u krajnjem slučaju lijekovi.
Ako je neko drugo zdravstveno stanje uzrok pojavi noćnog bruksizma, njegovim se otklanjanjem rješava i problem bruksiranja.

Abstract (english)

Glossary of Prosthodontic Terms defines bruxism as parafunctional grinding of teeth or an oral habit consisting of unvoluntary rhytmic or spasmodic non functional gnashing, grinding or clenhing of teeth in orther that chewing movements of the mandible which may lead to occlusal trauma.
Eventhough it is a frequent problem in population, etiology of bruxism is still not completely untied. A particular problem represents research of bruxism among children. A limiting problem are the clinical studies in which the examinees are juveniles.
The factors which are most often connected to bruxism are multiple. They are: personality of the person, especially anxiety and traits which point to stress, like also peripheral (morphological) factors (occlusion and orofacial anatomy).
These are the reasons why methods which have been used in the diagnosis of bruxism among children have to be multifactorial and include questionnaires and interviews with children and their parents, clinical checkups (intraoral and extraoral) and also in some cases EMG and polysomnography during sleep.
We can devide the consequences of bruxism on dental and others. The dental ones are: localized and general detrition of teeth, necrosis or fracture of a tooth, inflammation of gingiva and parodont. Some even state the premature eruption of permanent successors. Other consequences of night bruxism are hypertrophy of chewing muscles, headache, pain during chewing and disfunction of the temporomandibular joint.
