Abstract

Pušenje duhana i duhanskih proizvoda danas je veliki zdravstveni, ekonomski i socio-kulturni problem kako u svijetu, tako i u Hrvatskoj. Procjenjuje se da u svijetu duhanske proizvode puši 47% muškaraca i 12% žena te da godišnje od posljedica pušenja umire 5 milijuna ljudi. U Hrvatskoj je 2002. godine zabilježeno 34% muškaraca i 27% žena pušača. Dok s godinama na zapadu trend pušenja opada, u Hrvatskoj on raste. Naime, 2006. godine procjenjuje se da duhanske proizvode puši 36 % muškaraca i 27 %... More žena.
Poznato je da duhanski dim sadrži 4000 štetnih kemijskih sastojaka. Među njima kao najzastupljeniji ističu se dušik, ugljični dioksid, kisik, nikotin, ugljični monoksid, policiklički aromatski ugljikovodici, metali i cijanovodik.
Pušenje je čimbenik rizika za tri vodeće skupine uzroka smrti u Hrvatskoj 2005. godine – kardiovaskularne bolesti, maligne bolesti i bolesti respiratornog sustava – koje u ukupnoj smrtnosti sudjeluju s 81,1%, a uzrokovale su preko 200.000 izgubljenih godina života. Osim štetnog učinka na opće zdravlje, svakodnevno vidimo i posljedice pušenja na oralnim strukturama. Tu spadaju karcinom usne šupljine, prekanceroze, pigmentacije tvrdih zubnih tkiva te bolesti žlijezda slinovnica sa posljedičnom kserostomijom. Indiretan utjecaj pušenja na oralne strukture vidljiv je kroz lošu oralnu higijenu. Naime, pušači imaju lošiju higijenu koja onda igra odlučujući faktor u etiologiji karijesa i zubnog kamenca te nakupljanju plaka i nastanku gingivitisa i parodontitisa. Kao doktori dentalne medicine dužni smo pomoći svojima pacijentima u prestanku pušenja te u prevenciji i sanaciji oralnih stanja nastalih lošom higijenom potaknutom pušenjem. Less

Abstract (english)

Smoking tobacco and tobacco products is a major health, economic and socio-cultural problem both in Croatia and worldwide. It is estimated that 47% of men and 12% of women smoke tobacco worldwide and that 5 million people per year die from effects of smoking. In 2002, Croatia recorded 34% of men and 27% of women smokers. While the western trend of smoking decreases over the years, in Croatia it grows. In 2006, it is estimated that 36% of men and 27% women smoke tobacco. It is well known that... More tobacco smoke contains 4000 harmful chemical substances, the most represented being nitrogen, carbon dioxide, oxygen, nicotine, carbon monoxide, polycyclic aromatic hydrocarbons, hydrogen cyanide and metals. Smoking is a risk factor for the three leading causes of death in Croatia in 2005 - cardiovascular diseases, malignant diseases and diseases of the respiratory system -which accounted for 81.1% in total mortality, and caused more than 200000 years of life lost. In addition to the adverse effect on the general health, the consequences of smoking may also be seen on oral tissues. These include cancers of the oral cavity, premalignant lesions, pigmentation of hard dental tissue and salivary gland diseases that cause xerostomia. Furthermore, there is an indirect impact of smoking on oral structures through bad oral hygiene. Smokers have poorer oral hygiene which is a decisive factor in the etiology of dental carries and plaque accmulation, as well as emergence of gingivitis and periodontal disease. As doctors of dental medicine, we are obliged to help our patients stop smoking and help them prevent and treat oral conditions caused by poor hygiene induced by smoking. Less
