Abstract

Diabetes mellitus je jedna od najučestalijih metaboličkih bolesti karakterizirana nedostatkom inzulina ili su tjelesne stanice neosjetljive na učinak inzulina. Diabetes izaziva multisistemske učinke poput retinopatije, nefropatije, neuropatije i kardiovaskularnih bolesti koje značajno utječu na javno zdravlje. Diabetes može također uzrokovati akutne komplikacije koje zahtijevaju hitno liječenje, kao npr. ketoacidoza te dugotrajne komplikacije koje mogu zahtijevati operacijski postupak. Jako je bitno pažljivo pratiti razinu glukoze u krvi tijekom cijelog perioperacijskog perioda. Kronične komplikacije dijabetesa mogu dovesti do vrlo ozbiljnih mutilacija kod pacijenata kao npr. amputacije donjih udova. Uzrok tome su aterosklerotske promjene perifernih krvnih žila. Najučestalija komplikacija dijabetesa uzrokovana aterosklerotskim sužavanjem arterija i neuropatijom je dijabetičko stopalo koje
zahtijeva poseban kirurški tretman.

Abstract (english)

Diabetes mellitus is one of the most common metabolic diseases in which either the hormone insulin is lacking or the body's cells are insensitive to insulin effects. The multi-system effects of diabetes such as retinopathy, nephropathy, neuropathy and cardiovascular diseases are considered important impinging on the public health. Diabetes may also cause acute complications that require urgent treatment such as ketoacidosis, and long-term complications that require operative management. Meticulous attention to blood glucose monitoring is essential throughout the perioperative period. Long term complications of diabetes can lead to seriously mutilations of the patients such as amputation of the lower limbs. That is caused by peripheral vascular disease. The most frequent complication of diabetes, caused by both, atherosclerotic changes of arteries and neuropathy, is diabetic foot. Is requires specific surgical treatment.
