Abstract

Gubitak prirodnih zuba narušava izgled, govor, funkciju stomatognatog sustava i životni stil pojedinca. Unatoč naporima liječnika u podizanju svijesti o oralnoj higijeni i pravilnoj prehrani, vodeći je uzrok gubitka zuba i dalje karijes. Brojnim istraživanjima dokazana je povezanost incidencije djelomične bezubosti u populaciji s dobi i socio-ekonomskim parametrima. Pravovremena nadoknada izgubljenih zuba može smanjiti oštećenja stomatognatog sustava i složenost oralne rehabilitacije, a ovisi o razini svijesti pacijenta o oralnom zdravlju i vlastitom izgledu. Protetska sanacija složen je postupak koji je moguće uspješno izvesti jedino postavljanjem ispravne dijagnoze i pomnom izradom plana terapije. Kombinirani rad kao terapijsko rješenje predstavlja alternativu implantoprotetskoj terapiji i izradi fiksnog nadomjeska. Fiksno-mobilni nadomjestak sastoji se od fiksno cementirane konstrukcije, mobilne djelomične proteze i veznog elementa koji služi za retenciju dvaju dijelova. Fiksna konstrukcija kojom se saniraju uporišni zubi djeluje kao profilaktička zaštita i omogućuje povoljan raspored opterećenja. Precizni pričvrsci osiguravaju kvalitetnu retenciju, stabilizaciju i integraciju proteze u zubni luk u funkcijskom i estetskom smislu. U izradi kombiniranog rada postoji širok raspon potencijalnih pogrešaka zbog velikog broja kliničkih i laboratorijskih faza terapije. Određivanje međučeljusnih odnosa predstavlja jednu od najvažnijih terapijskih faza te joj je potrebno posvetiti pozornost. Ponekad ju je potrebno provesti već tijekom dijagnostičkog postupka. Nepravilno cementiranje završenog kombiniranog rada vodi do neuspjeha čitave terapije na samom kraju. Složenost planiranja i izvedbe ove vrste terapije zahtijeva kliničko iskustvo i stručnost doktora dentalne medicine te vještinu dentalnog tehničara s kojim je potrebno imati jasnu komunikaciju.

Abstract (english)

The loss of natural teeth impairs the appearance, speech, function of the stomatognathic system and the individual's lifestyle. Despite the efforts of doctors to raise awareness about oral hygiene and proper nutrition, the leading cause of tooth loss is still caries. Numerous studies have proven the correlation of the incidence of partial edentulism in the population with age and socioeconomic parameters. Replacement of lost teeth can reduce damage to the stomatognathic system and the complexity of oral rehabilitation, and it depends on the patient's level of awareness of oral health and personal appearance. Prosthetic rehabilitation is a complex procedure that can only be successfully performed by establishing a correct diagnosis and carefully creating a therapy plan. Combined work as a therapeutic solution represents an alternative to implant-prosthetic therapy and the creation of a fixed prosthesis. A fixed-mobile prosthesis consists of a fixed cemented structure, a mobile partial denture and a connecting element that serves to retain the two parts. The fixed structure used to restore the abutment teeth acts as prophylactic protection and enables a favorable load distribution. Precise attachments ensure high-quality retention, stabilization and integration of the prosthesis into the dental arch in a functional and aesthetic sense. There is a wide range of potential errors in the fabrication of combined prosthetics due to the large number of clinical and laboratory phases of therapy. Determining oclusal relations is one of the most important therapeutic phases to which attention must be paid. Sometimes it is necessary to perform it already during the diagnostic procedure. Improper cementation of the completed combined prosthetic leads to the failure of the entire therapy at its very end. The complexity of planning and execution of this type of therapy requires the clinical experience and expertise of a doctor of dental medicine and the skill of a dental technician with whom it is necessary to have clear communication.
