Abstract

U svakodnevnom radu stomatolog praktičar mora koristiti tehnike određivanja međučeljusnih odnosa kako bi protetska rehabilitacija bila uspješna. Referentni položaji koje koristimo kako bismo prenijeli odnos mandibule prema maksili u artikulatore, mehaničke naprave čija je glavna svrha što točnija i individualnija reprodukcija kretnji mandibule i odnosa među čeljustima, jesu CR i MI. Kao što se definicija CR kroz povijest mnogo puta mijenjala, tako su provedena i mnoga istraživanja kojima su testirane metode određivanja CR i MI. Osim pokušaja dobivanja vrijednosti spomenutih položaja, proučavan je i odnos CR-MI. Drugim
riječima, gledalo se podudaraju li se ta dva položaja, tj. postoji li među njima velika razlika. Dobiveni rezultati otkrili su da je razlika između CR i MI u rasponu od 1-2 mm, te kako se ta vrijednost smatra fiziološkom. No, ako je razlika veća od 2 mm,
upućuje nas na to da se radi o klasi II, ortodontskoj anomaliji, ili je etiologija nepoznata. U svakom slučaju, prisutna je ortopedska nestabilnost čija povećana vrijednost ukazuje na veću mogućnost razvoja TMP-a. Podudaranje položaja CR i MI javlja se u 10% populacije, a 100% smo sigurni da se ovi položaji podudaraju samo kod dobro izvedenih potpunih proteza. Uvijek je dobro poznavati više metoda određivanja međučeljusnih odnosa, ali je ipak bolje znati ispravno se služiti barem jednom od njih, jer su samo naša sigurnost i iskustvo preduvjet razvoja uspješne karijere i zadovoljstva pacijenata.

Abstract (english)

In his daily work, a dentist is required to use techniques of defining maxillomandibular relationships in order for the prosthetic rehabilitation to be successful. The referent positions that we use to transfer the relationship of a mandible towards a maxilla to an articulator, a mechanical device whose main purpose is accurate and individual reproduction of mandible movements and maxillomandibular relationships, are CR and MI. Since the definition of CR has changed repeatedly during history, methods of defining both CR and MI have been tested through numerous studies.Besides the attempts to determine the values of mentioned positions, the CR-MI correlation has also been studied. In other words, the two positions were monitored in order to establish to what level they correspond, or if there is any kind of variation between them. The results have shown that the variations between CR and MI range from 1-2 mm, the value considered to be physiological. The variations larger than 2 mm would point to the class II problem, an orthodontic anomaly, or to the unknown etiology. In any case, the increased value of the orthopedic instability points to the higher possibility of TMD development. The correspondence of CR and MI positions is noticeable in 10% of the population and can be with 100% certainty traced to the cases of total dentures. Although it is always useful to be familiar with different methods of defining maxillomandibular relationships, intimate knowledge of a single method is preferred because only our confidence and experience are an assurance of a successful career and the satisfaction of our patients.
