Abstract

Površinu prepariranog dentina pokriva tanak sloj dentalne piljevine sličnog kemijskog sastava kao i dentin, koji nazivamo zaostatni sloj. Klasični pristup preparaciji kaviteta nije nužno zahtijevao predtretman dentina u smislu uklanjanja ovog sloja. Početkom pedesetih godina adhezijski postupak ograničio se samo na kemijski tretman caklinskih rubova, što nije dalo posebno
dobre kliničke rezultate. Kemijska obrada dentina i vezivanje složenih materijala za dentin pokazali su se težom i složenijom zadaćom. Jedan od problema koji je trebalo riješiti bilo je uklanjanje zaostatnog sloja na što jednostavniji i klinički prihvatljiv način.Više od stotinu kliničkih istraživanja, stalan napredak i poboljšanja dentinskih materijala i adheziva, daju danas optimalne rezultate kako same kliničke procedure tretmana zaostatnog sloja tako i same čvrstoće veze ispuna naspram demineraliziranog
dentina. Biokompaktibilnost zubne pulpe naspram kemijskog predtretmana zaostatnog sloja i dentina i dalje se provjerava različitim parametrima bioloških testiranja, kako bi klinički uspjesi imali potporu i potvrdu. Suvremeni adhezijski sustavi u svom sastavu imaju kiselinu što smanjuje broj faza rada, skraćuje vrijeme rada te umanjuje mogućnost pogreške, no i dalje su predmet kliničkih ispitivanja u smislu poboljšanja njihovih svojstava.

Abstract (english)

Surface of exposed dentin, after removal off carries, is covered with smear layer. In the past it was not necessary to remove this layer. Different materials and techniques required different treatment of smear layer. Chemical treatment of dentin and adhesion of the composite restoration to dentin proved to be a challenging and complex task. One of the problems witch needed to be solved is the removal of smear layer by the simplest and clinically acceptable way. Constant progress and improvement of dentin materials and adhesives allow us to have good clinical result, in treating smear layer, same as achieving a good and solid bond between restorative material and tooth surface. Biocompatibility of dental pulp against the chemical pre-treatment and the use of adhesive system is constantly being examined by certain parameters of biological
testing in order to provide substantial support and confirmation of clinical achievements. New adhesives have acids as a component witch shortens the time of work required to apply them and reduces the possibility of mistake, but are still an object of clinical trials in the effort to improve their characteristics.
