Mogućnosti uporabe mini dentalnih implantata u stomatološkoj protetici

Abstract

Dentalni implantati su u uporabi dugi niz godina, a odnedavno je u uporabi i mini dentalni implantat (MDI). To je implantat promjera od 1,8 do 2,4 mm i upravo iz razlike u promjeru proizlaze sve njegove prednosti, odnosno mane. MDI nisu zamjena za
standardne implantate, nego imaju svoje usko indikacijsko područje. Uporaba MDI-a indicirana je u slučajevima smanjenog volumena kosti (nedostatan za uporabu standardnih implantata) te u frontalnim područjima mandibule i maksile kod nedostatka
jednog zuba (preuzak prostor za postavljanje standardnog implantata). Prednost uporabe MDI-a je i minimalna trauma kod implantacije uz imedijatno opterećenje. Postavlja se transgingivno uz minimalnu preparaciju kosti za njegovo ležište te se dalje sam uvija u neprepariranu kost (svojstvo samourezivanja). Primarni i sekundarni dio MDI-a čine jedinstvenu cjelinu te se meka tkiva odmah po implantiranju formiraju oko vrata implantata, time se skraćuje implantacijski postupak i odmah se pristupa izradi protetske suprastrukture. Osnovna indikacija za uporabu MDI-a je retencija donje totalne proteze. Tada se koriste minimalno četiri MDI-a, što je i financijski isplativije od uporabe minimalno dva standardna implantata. Kod osnovne indikacije rezultati uporabe MDI-a su ekvivalentni onima ostvarenim standardnim implantatima i očituju se boljom retencijom i stabilizacijom, boljom funkcijom, otklanjanjem teškoća u govoru i preosjetljivosti mekih tkiva ležišta proteza. Ugrañeni MDI-i imaju veliku primarnu stabilnost (samourezivanje) što im omogućava i dobru oseointegraciju, pa stopa „preživljavanja“ MDI-a iznosi čak 94,2%.

Abstract (english)

Dental implants have been used for years, but recently mini dental implant (MDI) has been introduced. It ranges from 1.8 mm to 2.4 mm in diameter and hence derive all of its advantages and disadvantages. MDI is not a substitute to standard-sized implant, but has its own narrow field of indications. The usage of MDI is indicated in cases of reduced bone volume (insufficient for usage of standard-sized implants) and in cases of single tooth missing in frontal area of both mandible and maxilla (residual space too narrow for standard-sized implant placement). Another advantage of MDI usage is minimal
placement trauma with immediate loading option. MDI is placed transgingivally with minimal bone preparation for its implant site and screwed later on into the bone using the self-tapping feature of the implant. Dental implant body and transmucosal abutment of MDI make a single unit which enables soft tissue to form itself around the neck of the implant immediately after insertion. This possibility shortens the implantation procedure and prosthodontic suprastructure can be made. Primary indication for usage of MDI is retention of complete lower prosthesis. This requires usage of minimaly 4 MDIs which is also financially more profitable than using two standard-sized implants. Primary indication for MDI has equivalent result to standard-sized implant and it manifests in better retention and stabilization, improved function, removal of speech difficulties and soft tissue hypersensitivity. Inserted MDI has great primar stability (self-tapping feature) wich enables a proper osseointegration resulting in MDI „survival“ rate of 94.2%.
