Abstract

Sve češće propisivanje antibiotika potaknulo je tijekom posljednjih desetak godina u mnogobrojnim znanstvenim krugovima polemiku o reviziji smjernica za antibiotsku profilaksu u dentalnoj medicini, i to jednako u medicinski kompromitiranih te zdravih pacijenata. Na sve češće propisivanje antibiotika snažno je utjecala i pandemija koronavirusa COVID-19. Takva situacija doprinijela je sve većoj rezistenciji bakterija na antibiotike čime se povećava i stopa smrtnosti oboljelih od sistemske infekcije odnosno sepse uzrokovane multirezistentnim patogenima. Američka udruga za srce (AHA) objavila je 2021. godine reviziju smjernica za antibiotsku profilaksu infektivnog endokarditisa, a posljednja revizija smjernica bila je 2007. U novoj reviziji smjernica nailazimo na mnogobrojne promjene u izboru antibiotika, odnosno napuštaju se određeni antibiotici zbog prevelikih komplikacija ili prevelike rezistencije. Također, mnogobrojni doktori dentalne medicine odlučuju se za antibiotsku suportivnu terapiju radi preveniranja neželjenih komplikacija pri postavi dentalnih implantata. Takva situacija navela je mnogobrojne znanstvenike na objavljivanje znanstvenih i preglednih radova te metaanaliza kako bi opovrgnuli teoriju neželjenih komplikacija i podučili doktore dentalne medicine u kojim situacijama, kojim pacijentima te u kojoj je dozi potrebno propisati antibiotsku profilaksu pri postavi dentalnih implantata, a sve u svrhu sprječavanja rastućeg trenda propisivanja antibiotika.

Abstract (english)

The increasingly frequent prescription of antibiotics over the last ten years has started a debate in numerous scientific circles about revising antibiotic prophylaxis guidelines in dental medicine, both in medically compromised as in healthy patients. The ever-increasing prescription of antibiotics was highly affected by the COVID-19 coronavirus pandemic. Such
situation has contributed to the higher bacteria resistance to antibiotics, increasing the mortality rate of patients suffering from systemic infection or sepsis caused by multi resistant pathogens. In 2021, the American Heart Association (AHA) published a revision of the antibiotic prophylaxis guidelines for infective endocarditis. The previous revision of the guidelines was in 2007. The new, revised guidelines introduce numerous changes in the choice of antibiotics, replacing certain antibiotics due to excessive complications or excessive resistance. Likewise, when placing dental implants, many Doctors of Dental Medicine prescribe antibiotic supportive therapy to prevent unwanted complications. This situation has prompted numerous scientists to
publish scientific papers, review articles, as well as conduct meta-analyses in order to disprove the theory of unwanted complications and advise Doctors of Dental Medicine in which situations, which patients and in which dosage it is necessary to prescribe antibiotic prophylaxis in dental implant surgery in order to prevent the further growing trend of prescribing antibiotics.
