Abstract

Procjena dentalne dobi sastavni je dio forenzičke dentalne medicine. U prenatalnoj, novorođenačkoj, dječjoj i adolescentnoj dobi procjena se temelji na proučavanju stadija razvoja tvrdih zubnih tkiva, dok se u odrasloj dobi promatraju degenerativne promjene
do kojih dolazi starenjem. Metode procjene dentalne dobi mogu se podijeliti na morfološke, radiološke i biokemijske. Najznačajnija promjena do koje dolazi starenjem smanjenje je volumena pulpne komore uslijed fiziološkog odlaganja sekundarnog dentina. Osim toga, kao reakcija na patološke podražaje odlaže se tercijarni dentin, a starenjem je uočena i povećana incidencija dentikala unutar endodontskog prostora. Procjena dentalne dobi na temelju promjena endodontskog prostora može se provoditi putem omjera pulpe i zuba te mjerenjem indeksa koronarne pulpe na rentgentskim snimkama. Kvaal i suradnici mjerili su duljinu zuba, pulpe i korijena te širinu korijena i pulpe u tri točke na periapikalnim snimkama šest zuba pa generirali formulu za procjenu dentalne dobi: dob=129.8 – 316.4M – 66.8(W – L). Cameriere i suradnici stavili su u omjer površinu zuba i pulpe, a razvojem CBCT-a osmišljena su i volumetrijska mjerenja za procjenu dentalne dobi. Drusini i suradnici koristili su Ikedinu formulu za procjenu dobi mjerenjem visine koronarne pulpe (CPCH) te visine krune zuba (CH) nakon čega je izračunat indeks koronarne pulpe putem formule TCI = CPCH x 100/CH. Metode procjene dentalne dobi na temelju promjena endodontskog prostora smatraju se neinvazivnima i jednostavnima, ali potrebna su daljnja istraživanja radi poboljšanja točnosti rezultata.

Abstract (english)

Dental age estimation is a big part of forensic odontology. Prenatal, newborn, children and adolescent dental age assesment is based on phases of development of the teeth, while in the adult dentition degenerative changes due to aging are observed. There are several dental age assesment methods: morphological, radiological and biochemical. The most
significant change that occurs with aging is the reduction of pulp cavity due to physiological secondary dentin deposition. Additionally, as a response to pathological stimuli, tertiary dentin is formed. Increased incidence of denticles within the endodontic space has also been observed. Dental age estimation based on pulp cavity changes can be carried out through the pulp-tooth ratio, as well as by measuring the coronal pulp cavity index. Kvaal et al. measured the lenght of tooth, pulp and root, as well as width of root and pulp on three different levels in periapical images of six teeth. Based on this, they
generated formula for dental age estimation: age=129.8 – 316.4M – 66.8(W – L). Cameriere et al. used pulp-tooth area ratio, and with the development of CBCT technology, volumetric measurements were introduced. Drusini et al. used a different approach using Ikedas formula for dental age estimation by measuring the height of coronal pulp cavity (CPCH) and the height of the tooth crown (CH). Tooth coronal index was calculated using the formula: TCI = CPCH x 100/CH. Dental age estimation methods based on changes of the endodontic space are considered non-invasive and straightforward, although further research is needed to enhance result accuracy.
