Abstract

Suvremena dentalna medicina stavlja naglasak na očuvanje tvrdih zubnih tkiva i upotrebu
poštednih metoda terapije, odnosno zauzima se za minimalo invazivni pristup. Ta velika
promjena u opsežnosti brušenja omogućena je primjenom adhezijskih sustava kojima zub i
nadomjestak postaju cjelina.
Minimalno invazivni nadomjesci su: ljuske, inleji, onleji, overleji, endokrunice i adhezivni
mostovi. Ljuske su nadomjesci koji korigiraju estetiku prednjeg segmenta zubnog niza
mijenjajući izgled vestibularne plohe istih. Koriste se za korekciju boje, oblika, nagiba i dužine
zuba te za zatvaranja dijastema. Inleji, onleji i overleji su indirektne intrakoronarne restauracije
koje imaju bolja mehanička i estetska svojstva od klasičnih direktnih restauracija. Endokrunice
su indicirane kod endodontski liječenih kutnjaka s velikim gubitkom tvrdih zubnih tkiva kod
kojih se preostalo tkivo nastoji sačuvati, a osobito kod onih zubi kod kojih je izrada intrakanalne
nadogradnje i krunice kontraindicirana. Adhezijski mostovi su nadomjesci indicirani za
nadomještanje jednog zuba u prednjem segmentu zubnog niza, a osobito kod pacijenata kod
kojih su dentalni implantati i konvencionalni dentalni mostovi kontraindicirani. Preparacija
zuba nosača je veoma poštedna, a najčešće se radi o privjesnom mostu i jednom zubu nosaču.
Zadaća doktora dentalne medicine je uz pravilnu indikaciju biti što manje invazivan u svojem
tretmanu, a istodobno zadovoljiti estetske i funkcijske zahtjeve profesije i pacijenta.

Abstract (english)

The modern dental medicine is putting emphasis on the preservation of hard dental tissue and
on using a minimally invasive approach. The big change in the amount of tooth preparation was
possible after the appearance of adhesive cementation which unifies the tooth and the
restoration.
Minimally invasive restorations in fixed prosthodontics are the following: veneers, inlays,
onlays, overlays, endocrowns and resin-bonded bridges. Veneers are restorations used for
aesthetic corrections of frontal teeth. They are used for changing the shape, inclination, length
or colour of the teeth, as well as for closing the widen proximal space. Inlays, onlays and
overlays are indirect intracoronal restorations with better mechanical and aesthetic properties
than conventional direct restorations. Endocrowns are used on endodontically treated molars
with severe loss of hard tissue for preserving the hard tissue that is left, and especially on teeth
in which the post and core are contraindicated. Resin-bonded bridges are used for replacing one
tooth in the frontal region, and especially in patients who have contraindications for dental
implants and dental bridges, such as young patients. Tooth preparation is minimally invasive
and often includes only one tooth.
A dentist should be as less invasive as possible, and at the same time he or she should satisfy
patient's aesthetic and functional expectations.
