Abstract

Oroantralna komunikacija je umjetno stvorena, patološka komunikacija između usne šupljine i
maksilarnog sinusa. Najčešći uzrok nastajanja oroantralne komunikacije je ekstrakcija gornjih
premolara i molara, iako može nastati i zbog patoloških procesa u periapeksnom području istih
zuba. Uz sve navedeno, postoje i određeni predisponirajući čimbenici koji dodatno povećavaju
mogućnost nastajanja oroantralne komunikacije. Pneumatizirani maksilarni sinus, dugi i
divergentni korijenovi zuba te neadekvatna, gruba tehnika ekstrakcije samo su neki od njih.
Dijagnostika se provodi Valsalvinim testom.
Nedijagnosticiranjem i neliječenjem oroantralna komunikacija dovodi do nastanka komplikacija u
vidu oroantralne fistule, koja perzistiranjem uzrokuje nastanak maksilarnog sinusitisa uslijed
ulaska hrane i patogenih čimbenika iz usne šupljine u maksilarni sinus. U svrhu sprečavanja
navedenih komplikacija iznimno je važno poznavati ih te znati kako liječiti samu oroantralnu
komunikaciju.
Liječenje oroantralne komunikacije potrebno je provesti odmah nakon dijagnosticiranja. Postupak
liječenja najviše ovisi o veličini same komunikacije, vremenu koje je prošlo od nastanka i o
prisutnosti upale maksilarnog sinusa. Raznim kirurškim zahvatima liječe se veće komunikacije dok
je temelj liječenja manjih komunikacija potpomaganje stvaranja i održavanja krvnog ugruška u
alveoli.

Abstract (english)

Oroantral communication is artificially created, pathological communication between the oral
cavity and the maxillary sinus. The most common cause of oroantral communication is the
extraction of the upper premolars and molars, although it can also occur due to pathological
processes in the periapical area of the same teeth. In addition to all the above, there are certain
predisposing factors that increase further the possibility of oroantral communication. Pneumatized
maxillary sinus, long and the divergent teeth roots, and inadequate, rough extraction technique are
just some of them.
Diagnosis is performed with the Valsalva test.
By not diagnosing and not treating, oroantral communication leads to the complications in the form
of oroantral fistula, which leads persistently to the development of maxillary sinusitis due to the
entry of food and pathogenic factors from the oral cavity into the maxillary sinus. In order to
prevent these complications, it is extremely important to know them and know how to treat
oroantral communication itself.
The treatment of oroantral communication should be carried out immediately after its diagnosis.
The treatment procedure depends mostly on the size of the communication itself, the time that has
elapsed since its onset and the presence of the inflammation of the maxillary sinus. Larger
communications are treated with various surgical procedures, while the basis of the treatment of
smaller communications is to support the formation and the maintenance of a blood clot in the
alveoli
