Abstract

Implantoprotetika ima sve veću popularnost kao metoda nadomještanja izgubljenih zubi te rehabilitacije djelomično i potpuno bezubih pacijenata. Mobilni koncept implantoprotetske terapije potpune bezubosti čine pokrovne proteze retinirane implantatima. One osiguravaju bolju funkciju i estetiku u slučajevima velikog gubitka tvrdih i mekih tkiva, loših međučeljusnih odnosa (MČO) i visoke linije osmijeha. U slučajevima minimalnog ili nikakvog gubitka tvrdih i mekih tkiva, dobrih MČO te niske linije osmijeha, fiksni koncept bolja je terapija izbora. U fiksni koncept implantoprotetske terapije potpune bezubosti ubrajamo mostove na implantatima te fiksne proteze. Mostovi na implantatima estetski su prihvatljivi kod minimalnog gubitka kosti i mekih tkiva, što je rijetko slučaj kod dulje bezubih pacijenata. Stoga su fiksne proteze puno češće u kliničkoj praksi. Iako je izrada fiksnih
proteza kompliciranija te financijski zahtjevnija, bolje je prihvaćena od pacijenta zbog poboljšane retencije i stabilnosti te izostanka potrebe skidanja proteze iz usne šupljine. Tijekom godina stomatolozi su razvili brojne tehnike i njihove modifikacije za ugradnju dentalnih implantata u bezube čeljusti. Najpoznatije su tehnike All-on-4, All-on-6 i Trefoil tehnika. U ovom radu prikazana su tri klinička slučaja implantoprotetske terapije bezube čeljusti. Prvi je klinički slučaj implantoprotetska terapija fiksnom protezom uz korištenje All-on-4 tehnike. Drugi i treći klinički slučaj prikazuju implantoprotetsku terapiju pokrovnom protezom retiniranom prečkama na implantatima. U usporedbi s rehabilitacijom bezube čeljusti mobilnim potpunim protezama,
implantoprotetska terapija bezube čeljusti ostvaruje bolju retenciju i stabilizaciju, funkciju i estetiku.

Abstract (english)

Implant prosthetics is increasing in popularity as a method of substitution of missing teeth and rehabilitation of partially and completely edentulous patients. Mobile concept of implant prosthetic therapy of complete edentulism consists of implant - supported overdentures. They provide better function and aesthetics in cases of great loss of hard and soft tissues, bad
intermaxillary relations and a high smile line. In cases of minimal loss of hard and soft tissues, good intermaxillary relations and a low smile line, fixed concept is a better choice of therapy. Fixed concept of implant prosthetic therapy consists of implant bridges and full - arch fixed prosthesis. Implant bridges are aesthetically more acceptable in cases of minimal
bone and soft tissue loss, which is rarely the case in long - term edentulous patients. For that reasons, full – arch fixed prosthesis is more frequent in clinical practice. Although the production of a full – arch fixed prosthesis is more complicated and financially more demanding, it is more frequently accepted by the patient because of improved retention and
stabilization and the lack of need for removing the prosthesis from the oral cavity. Over years dentists developed numerous techniques and their modifications for implant placement in edentulous jaws. The most commonly known techniques are All-on-4, All-on-6 and Trefoil technique. In this paper, three clinical cases of implant prosthetic therapy were shown. First clinical case shows an example of implantoprosthetic therapy with a full – arch fixed prosthesis using Allon-4 method. Second and third clinical case show an example of implant prosthetic therapy using bar – retained and implant – supported overdenture.
In comparison of rehabilitation of edentulous jaw with complete removable dentures, implantoprostethic therapy achieves better retention and stabilization, function and aesthetics.
