Abstract

Parodontološka kirugija dio je druge korektivne faze parodontološke terapije, koja uz pomoć operativnih zahvata, nastoji očuvati parodontno zdravlje. Dijeli se na: resektivnu, regenerativnu i mukogingivalnu kirurgiju (kojoj su ciljevi očuvanje pričvrsne gingive,
zaustavljanje recesija i povećanje dubine vestibuluma). Mukogingivalna kirurgija uključuje zahvate mekog i tvrdog zubnog tkiva usmjerenih k augmentaciji gingive, prekrivanju površine korijena, korigiranju mukoznih defekata oko implantata, produljenju kliničke krune, očuvanju gingive na mjestima ektopičnog nicanja zuba, frenektomiji, frenotomiji, prevenciji kolapsa zubnog luka nakon vađenja zuba te augmentaciji bez zubnog grebena. Navedeni zahvati su od sekundarne važnosti za samo parodontno zdravlje. Razlog njihovog sve češćeg provođenja je želja stomatoloških pacijenata za prikrivanjem i najmanjih nepravilnosti u usnoj šupljini. Upravo iz estetskih razloga, danas se najčešće provodi prekrivanje gingivalnih recesija. Zahvaljujući razvoju parodontologije, njihovo prekrivanje moguće je uz razne metode. Istraživanja su pokazala kako je upotrebom vezivno-tkivnog transplantata uz koronalno pomaknuti režanj dobivena najbolja dugoročna stabilnost njihovog prekrivanja. Veliku ulogu u kakvoći dugoročnih rezultata ima mnoštvo faktora. Neki od njih su: oralna higijena, starenje, pravilna tehnika četkanja, širina keratinizirane gingive i nedostatak interdentalnog parodontnog tkiva prije samog zahvata. Svrha je ovog rada proučiti kojom se tehnikom dobivaju najbolji
rezultati te vidjeti jesu li oni održivi kroz dug period.

Abstract (english)

The periodontal surgery is divided into the resective, the regenerative and the mucogingival surgery, the goals of which are to preserve the attached gingiva, stop recessions and increase the depth of the vestibule. The mucogingival surgery includes soft and hard tissue dental procedures aimed at the augmentation of the gingiva, covering the root surface, the correction of mucosal defects around the implant, the lengthening of the clinical crown, the preservation of the gingiva at the sites of ectopic eruption, frenectomy, frenotomy, the prevention of the dental ridge collapse after the extraction and augmentation of the edentulous ridge. These procedures are of secondary importance for periodontal health. The reason for their increasingly frequent usage is the desire of dental patients to cover up even the smallest irregularities in the oral cavity. Because of such aesthetic reasons, the coverage of gingival recessions is the most often carried out procedure
nowadays. Thanks to the development of periodontology, their coverage is possible with various methods. Studies have shown that the use of a connective tissue graft with a coronally advanced flap technique resulted in the best long-term stability. Many factors play a big role in the quality of long-term results, some of which are oral hygiene, aging, a proper brushing technique, the width of the keratinized gingiva, and a lack of interdental periodontal tissue before the procedure itself. The purpose of this paper is to examine which technique gives the best results and to see if they are sustainable over a long period of time.
