Abstract

Nakon ekstrakcije zuba dolazi do promjena u horizontalnoj i vertikalnoj dimenziji alveolarnog grebena koje ugrožavaju buduću implantološku terapiju. U današnje vrijeme postoje mnogi augmentacijski materijali kojima se dimenzionalne promjene pokušavaju reducirati i na taj način poboljšati rezultati implantološke terapije. Premda postoje mnogi augmentacijski materijali koji se koriste, do danas se zlatnim standardnom smatra autogeni koštani transplantat zbog svojih osteoinduktivnih i osteokonduktivnih svojstava. Postoje neki problemi s autogenim koštanim graftom, kao što su infekcije na donatorskom mjestu, ograničena količina dostupne koštane mase i brza resorpcija autogene kosti. Zbog svega navedenog, zadnjih godina radi se
na istraživanju dentina kao alternativne zamjene autogenoj kosti. Mineralni i organski sastav zuba gotovo je identičan sastavu kosti. Demineralizirani dentin sadrži mrežu kolagena tip I s faktorima rasta koji potiču stvaranje kosti i služi kao dobar kalup za urastanje osteoblasta. On posjeduje i osteoinduktivna i osteokonduktivna svojstva. Stvoreni su novi uređaji, pomoću kojih se usitnjavaju zubi i pripremaju dentinski augmentativi u vremenskom razdoblju od 15 do 20 minuta od ekstrakcije zuba. Dentinski autologni graft može se koristiti na više načina: kao graft u podizanju dna sinusa, vođenoj regeneraciji tkiva, augmetaciji grebena i prezervaciji alveole. Također, on se osim kao prah može koristiti i kao blok kod većih koštanih defekata. Dentin predstavlja korisnu zamjenu drugim graftovima: posjeduje biokomatibilnost, ne izaziva imunološke reakcije kod pacijenta, sličnog je sastava kao kost koja se i smatra idealnim graftom. Zbog svega navedenog, njegova primjena u različitim augmentacijskim tehnikama
postala je sve više primjenjiva.

Abstract (english)

After tooth extraction, there are changes in the horizontal and vertical dimensions of the alveolar ridge that jeopardize future implant therapy. Nowadays, there are many augmentation materials that try to reduce dimensional changes and thus improve the results of implant therapy. Although there are many augmentation materials used to date, the gold standard is
considered to be the autogenous bone graft because of its osteoinductive and osteoconductive properties. There are some problems with autogenous bone graft, such as infections at the donor site, limited amount of available bone mass, and rapid resorption of autogenous bone. Due to all of the above, dentine research has been underway in recent years as an alternative to autogenous bone. The mineral and organic composition of the tooth is almost identical to the composition of the bone. Demineralized dentine contains a type I collagen network with growth factors that stimulate bone formation and serves as a good mould for osteoblast growth. It possesses both osteoinductive and osteoconductive properties. New devices have been created, which are used to grind teeth and prepare dentinal augmentatives in a period of 15 to 20 minutes from tooth extraction. Dentine autologous graft can be used in several ways: as a graft in raising the bottom of the sinus, in guided tissue regeneration, ridge augmentation and alveolar preservation. Also, instead of being a powder, it can be used as a block for larger bone defects. Dentine is a useful replacement for other grafts: it has biocompatibility, it does not cause immune reactions in the patient, it has a similar composition as bone, which is considered an ideal graft. Due to all the above, its application in various augmentation techniques has become increasingly applicable.
