Abstract

Bruksizam se opisuje kao patološko stanje obilježeno škrgutanjem, mljevenjem i stiskanjem zubi. Ponavljanje parafunkcijskih aktivnosti dovodi do trošenja tvrdog zubnog tkiva, glavobolja, boli u temporomandibularnom zglobu i pojave osjetljivosti zuba. Može se podijeliti u dva tipa, dnevni i noćni bruksizam. Pacijenti koji boluju od bruksizma većinom nisu svjesni svoga stanja
te se tek nakon pojave znatnije boli javljaju doktoru dentalne medicine. Bruksizam se najčešće dijagnosticira u ordinaciji dentalne medicine pomoću različitih upitnika, detaljne anamneze i kliničkog pregleda. Pacijenti se najčešće žale na bol u žvačnim mišićima i temporomandibularnom zglobu ili na pojavu glavobolja nakon spavanja. U terapiji bruksizma nude se različite terapijske mogućnosti. Budući da je stanje povezano sa stresnim situacijama i anksioznosti, preporuka je uklanjanje stresnih čimbenika. Najčešći je oblik terapije svakako korištenje stabilizacijskih udlaga kojima se onemogućuje daljnje trošenje
tvrdog zubnog tkiva i smanjuje se aktivnost žvačnih mišića. U nadoknadi potrošenog zubnog tkiva okreće se protetskoj rehabilitaciji. Protetska rehabilitacija mora biti pomno isplanirana jer se tijekom i nakon terapije mogu javiti brojne komplikacije. Zbog djelovanja jakih sila protetski je nadomjestak izbora solo krunica koja mora biti izgrađena od materijala koji će to opterećenje moći podnijeti. Kliničko iskustvo doktora dentalne medicine ima najveću ulogu u izboru materijala. Nakon izrade protetskih nadomjestaka, preporuka je pacijentima dati stabilizacijske udlage, ali njihova djelotvornost nije potvrđena.

Abstract (english)

Bruxism is described as a pathological condition characterized by grinding and/or clenching of teeth. Repetition of parafunctional activities leads to tooth wear, headaches, pain in the temporomandibular joint and tooth sensitivity. It can be divided into two types: daytime and nighttime bruxism. Patients suffering from bruxism are mostly unaware of their condition and contact their dentist only after the onset of significant pain. Bruxism is commonly diagnosed in dental offices using a variety of questionnaires, detailed medical history, and clinical examination. Patients most often complain of pain in the
masticatory muscles and temporomandibular joint or headaches after sleep. In the therapy of bruxism, various therapeutic possibilities can be chosen. As the condition is associated with stressful situations and anxiety, it is recommended to remove stressors. The most common therapy is certainly the use of stabilizing splints that prevent further wear of the
hard tooth tissue and reduce the activity of the masticatory muscles. In the compensation of tooth wear, there is prosthetic rehabilitation. Prosthetic rehabilitation must be carefully planned as numerous complications can occur during and after the therapy. Due to the action of strong forces, the prosthetic replacement of choice is a solo crown that must be built of a material that will be able to withstand the load. The clinical experience of the dentist plays the biggest role in the choice of materials. After making prosthetic replacements, it is recommended to give patients stabilizing splints, but their effectiveness has yet not been confirmed.
