Abstract

Periimplantatne bolesti multifaktorijalne su etiologije i neki pacijenti su podložniji njihovom
nastanku. Primarni uzrok periimplantatnih bolesti je oralni biofilm dok određeni čimbenici
rizika mogu ubrzati razaranje periimplantatnih tkiva. Periimplantatna tkiva imaju smanjenu
otpornost na mehaničke i mikrobiološke utjecaje u usporedbi s parodontnim tkivima. Postoje
snažni dokazi da povećan rizik za razvoj periimplantatnih bolesti imaju pacijenti s
uznapredovalim parodontitisom, neadekvatnom kontrolom plaka i bez redovite terapije
održavanja nakon ugradnje dentalnih implantata. Rezultati znanstvenih istraživanja o diabetesu
mellitusu i pušenju, kao potencijalnim indikatorima odnosno čimbenicima rizika, nisu dovoljno
uvjerljivi. Periimplantatni mukozitis upalna je lezija periimplantatnih mekih tkiva bez gubitka
marginalne kosti oko implantata, a periimplantitis patološko stanje karakterizirano upalom
periimplantatnih mekih tkiva i progresivnim gubitkom potporne kosti oko implantata. Pušenje
utječe na rast oralnih patogenih mikroorganizama direktnim utjecajem na mikrookoliš ili
indirektno utjecajem na imunološki odgovor domaćina. Pušenje sprječava regeneraciju
periimplantatnih vezivnih tkiva i kosti. Klinički znakovi upale kod pušača potisnuti su štetnim
utjecajem duhanskog dima na imunološki odgovor domaćina i vaskularizaciju i oksigenaciju
tkiva. Rezultati znanstvenih istraživanja sugeriraju da pušenje povećava rizik neuspjeha
dentalnih implantata i gubitka marginalne kosti oko implantata te pridonosi nastanku
postoperativnih infekcija. Još uvijek ne postoje jednoznačni dokazi da pušenje predstavlja
čimbenik rizika za razvoj periimplantatnih bolesti.

Abstract (english)

Peri-implant diseases are of multifactorial etiology and some patients are more susceptible to
their development. The primary cause of peri-implant diseases is oral biofilm, while certain risk
factors may accelerate the destruction of peri-implant tissues. Peri-implant tissues have reduced
resistance to mechanical and microbiological influences compared with periodontal tissues.
There is strong evidence that patients with advanced periodontitis, inadequate plaque control,
and without regular maintenance therapy after dental implant placement have an increased risk
of developing peri-implant diseases. Scientific data on diabetes mellitus and smoking as
potential indicators or risk factors are inconclusive. Peri-implant mucositis is an inflammatory
lesion of the peri-implant soft tissues without the loss of marginal bone around the implant, and
peri-implantitis is a pathological condition characterized by inflammation of the peri-implant
soft tissues and progressive loss of the supporting bone around the implant. Smoking directly
affects the growth of oral pathogenic microorganisms by affecting the microenvironment. It
also affects it indirectly by influencing the immune response of the host. Smoking inhibits the
regeneration of peri-implant connective tissues and bone. Clinical signs of inflammation in
smokers are suppressed by the detrimental effect of tobacco smoke on the host immune
response and tissue vascularization and oxygenation. The results of scientific research suggest
that smoking increases the risk of dental implant failure as well as the loss of marginal bone
around the implant and contributes to the development of postoperative infections. There is still
no conclusive evidence that smoking is a risk factor for the development of peri-implant
diseases.
