Abstract

U modernoj dentalnoj medicini implantološka terapija, kao i njezini brojni izazovi, dio su
svakodnevne kliničke prakse. Znamo da je za njezin uspjeh važna oseointegracija implantata i
optimalan protetski nadomjestak, a u posljednje se vrijeme sve više skreće pozornost na prijelaznu
zonu između implantata i suprastrukture te na njezinu ulogu u očuvanju zdravlja periimplantatnih
tkiva, mogućnosti uklanjanja plaka i krajnjem estetskom rezultatu implantoprotetskog rada
(crveno-bijela estetika). Tu tranzicijsku zonu čini transgingivni dio implantatne nadogradnje koji
mora biti odgovarajućeg oblika, kako bi pružio potporu ili čak oblikovao periimplantatna meka
tkiva te ostvario prijelaz između okruglog presjeka vrata implantata prema obliku kakav bi
topografski trebao imati zub koji nadoknađujemo.
Svrha rada je pregledno prikazati kakav utjecaj na izgled tranzicijske zone implantata imaju
pozicija i promjer implantata, širina pojasa keratinizirane sluznice i debljina sluznice, veza između
implantata i nadogradnje i slično. Nadalje, opisat će se tehnike modeliranja mekih periimplantatnih
tkiva s pomoću individualiziranih implantatnih nadogradnji ili vijaka za cijeljenje te tehnike
prijenosa postignutog oblika tranzicijske zone na konačan protetski rad.
Uvidom u recentnu literaturu prikazat ćemo kojim se metodama može na predvidljiv i dugoročno
stabilan način postići optimalna tranzicijska zona implantata u estetskoj, ali i posteriornoj regiji u
slučajevima imedijatne implantacije s imedijatnim opterećenjem, kao i kod odgođenog
opterećenja.

Abstract (english)

Implant therapy has become a part of daily practice in modern dental medicine requiring clinitians
to quickly respond to many of its challenges. Even though it is well known that oseointegration is
an important factor of success in implant therapy together with optimal prosthetic restorations,
greater focus has lately been put on the transitional zone between the implant and the
suprastructure. Its role is in preserving peri-implant health, enabling plaque removal and the final
aesthetic result of implantprosthetic therapy (pink and white aesthetics). The transitional zone is
the transgingival part of implant abutment which needs to be adequately shaped to support or even
shape the peri-implant soft tissue and make the transition between the round implant neck diameter
and a tooth profile in the specific dental ridge area.
The aim of this paper is to show the effect of various elements on the transitional zone appearance,
such as the position and the diameter of the implant, keratinized tissue width and mucosal
thickness, implant to abutment connection, etc. Furthermore, the techniques of peri-implant tissue
shaping by using individualized implant abutments or healing abutments will also be presented as
well as the techniques of transferring the emergence profile on definitive restauration.
The overview of recent literature will examine the methods providing predictable and long-term
stable result in shaping the transitional zone in both the aesthetic as well as the posterior region in
cases of immediate implant placement with immediate loading or in delayed implant
provisionalisation.
