Abstract

Parodontne bolesti su infektivne bolesti i jedan od glavnih uzročnika gubitka zubi u današnje vrijeme. Eliminacija patogenih mikroorganizama i mikrobiološkog biofilma, koji su glavni uzročnici ovih bolesti, glavni je cilj parodontne terapije. Terapijom se želi postići biokompatibilna površina korijena i ponovna kontrola imunološkog sustava nad mikroorganizmima usne šupljine.
Nekirurška parodontna terapija obično je prvi korak u zaustavljanju progresije bolesti i poboljšanju stanja parodonta pacijenta. Ona uključuje mehanički i kemoterapijski pristup u rješavanju problema. Mehanička se terapija odnosi na supragingivno uklanjanje plaka i kamenca te subgingivno čišćenje i poliranje korijena zuba koristeći ručne ili strojne instrumente. Antibiotici i antiseptici se koriste kao dodatna sredstva u borbi protiv parodontopatogenih mikroorganizama, osobito s područja koja su nedostupna mehaničkoj terapiji.
Zbog nedostataka klasične nekirurške parodontne terapije kao što je dugo trajanje tretmana, potrebna vještina za rukovanje kiretama, potrebno anesteziranje područja liječenja te nemogućnost pristupa određenim područjima, razvila se potreba za novim instrumentima i načinima liječenja parodontnih bolesti. Sve se češće koriste raznovrsni laseri, antimikrobna fotodinamska terapija, novi ultrazvučni instrumenti u terapiji zvanoj guided biofilm therapy (GBT) te ozon. Kako se u dosadašnjim istraživanjima ni jedan novitet nije pokazao boljim od klasične mehaničke terapije, potrebna su daljnja usavršavanja moderne tehnologije.

Abstract (english)

Periodontal diseases are infectious diseases and are the most common cause of tooth loss in the modern world. The main aim of periodontal therapy is to minimize or eliminate microbial biofilm, the primary cause of gingivitis and periodontitis. The endpoint of root debridement is to produce a surface that is biologically acceptable for healthy attachment and to enable imune system to overcome pathogen bacteria.
Nonsurgical periodontal therapy is usually the first step in stopping the progression of the disease and improving the patient's periodontal condition. It includes mechanical and chemotherapeutic approaches. Mechanical therapy refers to both supragingival and subgingival scaling and debridement of the roots by use of hand or power - driven instruments. Antibiotics and antiseptics are used for reducing periodontal pathogens from the entire oropharyngeal cavity.
Classic nonsurgical periodontal therapy has several disadvantages. It requires good manual skills, a considerable amount of time, anesthesia and is occasionally hampered by limited accessibility to some areas, etc. As a result, new instruments and methods for periodontal treatment have been developed and tested: various lasers, antimicrobial photodynamic therapy, new ultrasound instruments in the so - called guided biofilm therapy (GBT) and ozone.
These novelties, however, have not yet been proven superior to conventional mechanical therapy. Modern technological solutions in this area need to be further developed and perfected.
