Abstract

Dokazana uspješnost dentalnih implantata revolucionirala je protetsku terapiju i pridonijela funkcijskoj, estetskoj, i psihološkoj rehabilitaciji pacijenata nakon gubitka zubi. Zbog nedostatka parodontnog ligamenta, dentalni implantati biomehanički drugačije reagiraju od zuba za vrijeme opterećenja. Prilikom planiranja implantoprotetske sanacije potrebno je poznavati biomehaničke principe implantata. Svakog pacijenta treba promatrati kao zaseban slučaj te koristiti smjernice iz literature kako bi se zadovoljila funkcija i estetika, a mogućnost komplikacija svela na najmanju razinu. Definiranje okluzijskih shema i okluzijskog koncepta implantatima nošenih radova iznimno je važno te zahtijeva razmatranje. Pri planiranju okluzije u sklopu implantoprotetske terapije treba uzeti u obzir sve bitne činjenice i držati se smjernica iz literature. U literaturi ne postoji konsenzus koji bi izabrao točno određeni koncept okluzije za implantoprotetske nadomjeske. Razlog tomu je nedostatak znanstvenih dokaza. Većina današnjih smjernica bazira se na konceptu implantat-zaštićene okluzije te uzajamno zaštićene okluzije prilikom funkcijskih kretnji donje čeljusti. Dobrim planiranjem okluzije i dizajnom protetske suprastrukture izbjegavaju se rizični faktori koji dovode do okluzijskog preopterećenja. Ovaj rad opisuje smjernice za dizajn okluzijskih shema različitih tipova implantoprotetskih radova uzimajući u obzir bitne čimbenike kako bi se izbjegao nastanak okluzijskog preopterećenja i vezanih komplikacija.

Abstract (english)

The proven success of dental implants has revolutionized prosthetic therapy and contributed to the functional, aesthetic, and psychological rehabilitation of patients after tooth loss. Due to the lack of a periodontal ligament, dental implants biomechanically respond differently, compared to teeth, during occlusal loading. For planning implant-prosthetic rehabilitation, it is necessary to know biomechanical principles for dental implants. Therapy plan for each patient should be approached as an individual case and literature guidance should be used to satisfy function and aesthetics, minimizing the potential overloading and other complications. Defining occlusal schemes and the occlusal concept for implant supported prosthetic rehabilitation is extremely important and requires consideration. All relevant facts should be considered and guidelines from the literature should be followed as part of planning occlusion for implant supported prosthetics. There is no consensus in the literature on a specific occlusion concept for implant prosthetic restorations. The reason for this is the lack of scientific evidence. Most of today's guidelines are based on the concept of implant-protected occlusion and mutually protected occlusion during functional movements of the lower jaw. With good occlusion planning and good prosthetic superstructure design, we can avoid the risk factors that lead to occlusal overload. This paper describes the guidelines for the design of occlusion schemes for different types of implant prosthetics, taking into account important factors to avoid occlusion overload and the complications it carries.
