Abstract

Glavni je cilj endodontske terapije što duže sačuvati zub u usnoj šupljini, prevenirajući razvoj upalnih promjena pulpe sa posljedičnim periapikalnim lezijama te potičući uvjete za cijeljenje već postojećih lezija. To podrazumijeva uklanjanje inficiranog sadržaja te pravilnu obradu korijenskog kanala, njegovu dezinfekciju i konačno punjenje do internog foramena (lat. foramen physiologicum). Instrumente za obradu korijenskih kanala najjednostavnije možemo podijeliti na ručne i strojne. Nasuprot ručnim instrumentima izrađenim od nehrđajućeg čelika, uvođenje nikal titanske (NiTi) legure u stomatologiju dovodi do znatno lakše, sigurnije te preciznije instrumentacije i obrade korijenskih kanala. NiTi legure zbog svojih mehaničkih svojstava superelastičnosti i mogućnosti memorije postaju nezamjenjiv materijal za izradu strojnih instrumenata. Do danas su razvijeni brojni sistemi za strojnu instrumentaciju, počevši od rotacijskih do novijih tehnika koje koriste recipročne kretnje za obradu korijenskih kanala. Svaka od njih ima svoje prednosti i mane, no nijedna nije u potpunosti svladala sve poteškoće s kojima se susrećemo pri obradi korijenskih kanala.

Abstract (english)

The main goal of endodontic therapy is to preserve the tooth in the oral cavity for as long as possible, thus preventing the development of pulp inflamation and periapical lesions and stimulating the conditions for healing of already existing lesions. This involves removing the infected pulp,efficient root canal treatment, its disinfection and finally, filling up to the apical foramen (lat. foramen physiologicum). Instruments for root canal treatment can simply be divided into hand and motorized ones. The introduction of nickel-titanium alloy (Nitinol/NiTi) into dentistry leads to much easier, safer and more precise instrumentation and root canal treatment, as opposed to hand stainless-steel instruments. Owing to their mechanical properties of superelasticity and memory capabilities, NiTi alloysare becoming irreplaceable material for the production of motorized instruments. To date, a number of systems for mechanical instrumentation have been developed, ranging from rotary to newer techniques that utilize reciprocating movements for root canal treatment.Each has its own advantages and disadvantages, but none has completely overcome all the difficulties encountered in the treatment of root canal.
