Abstract

Konvencionalne totalne proteze retinirane isključivo ventilnim učinkom u većini slučajeva nisu dostatne osigurati dovoljnu retenciju i stabilizaciju koja je potrebna da bi se očuvala zadovoljavajuća žvačna funkcija. Osim žvačne funkcije, narušeni su i pacijentovo samopouzdanje te socijalna interakcija. Moderna dentalna medicina problemu bezubosti pristupa implantoprotetskom terapijom – koristi se dentalnim implantatima kao sidrištima za nadoknadu cijeloga zubnog luka. Ovisno o stupnju resorpcije alveolarnoga grebena, međučeljusnim odnosima i zahtjevima pacijenta, postoji više implatoprotetskih rješenja.
Pokrovna proteza poduprta implantatima mobilni je nadomjestak koji se retencijskim elementima sidrenima na implantatima koristi za postizanje bolje retencije i stabilizacije. Dostupan je niz retencijskih elemenata – pričvrsni sustavi u obliku kugličnih spojki i lokatora, prečke ili teleskopski sustavi. Pokrovnom je protezom moguće nadoknaditi prisutnost velikih resorpcija i međučeljusna neslaganja. Fiksna proteza poduprta implantatima fiksni je nadomjestak koji se vijčanom vezom spaja s implantatima te ga pacijent samostalno ne može ukloniti. Osim zubnog niza, u mogućnosti je nadoknaditi i malu do srednju resorpciju grebena. Most nošen implantatima također je fiksno rješenje, zahtijeva veći broj implantata te se fiksira vijčanom vezom ili cementom.
Svako navedeno rješenje ima svoje prednosti i nedostatke, no jedno je sigurno – primjena implantata u liječenju bezubosti danas je „zlatni standard“.

Abstract (english)

Conventional total dentures exclusively retained by valve effect are not, in most cases, adequate to secure sufficient retention and stabilization which is needed to preserve an adequate chewing function. Besides the chewing function, the patient’s self-confidence and social interaction are also impaired. Modern dental medicine approaches the edentulism by implant prosthetics therapy – it uses dental implants as anchors to compensate for the whole dental arch. Depending on the level of resorption of the alveolar ridge, intermaxillary relation and the patient’s demands, there are multiple implant prosthetic solutions available.
The overdenture supported by implants is a mobile appliance that uses the retention elements anchored on the implants for achieving better retention and stabilization. A range of retention elements is available – fastening systems like balls and locators, bars or telescopic systems. With overdentures it is possible to compensate for the presence of large resorptions and intermaxillary disproportions. Fixed dentures supported by implants are a fixed appliance that connects to the implants via screws and can not be removed by the patient independently. In addition to the reconstruction of the full arch they are capable of substituting small to medium sized ridge resorption. The bridge carried by implants is also a fixed solution, it requires a larger number of implants and is fixed by screws or cement.
Every listed solution has its strengths and weaknesses, but one thing is certain – using implants in edentulous therapy is currently the gold standard.
