Abstract

Kost posjeduje iznimnu regenerativnu sposobnost pa većina ozljeda cijeli bez nastanka ožiljkastog tkva. Međutim, različiti lokalni i sitemski uzroci mogu kompromitirati koštano cijeljenje i kao rezultat imati nastanak koštanih defekata. U takvim situacijama kirurškim metodama može se potaknuti koštana regeneracija.
Autogena i alogena kost, ksenogeni transplantati i aloplastični materijali samostalno ili u kombinaciji s membranama u tehnici vođene regeneracije kosti (engl. guided bone regeneration, GBR) materijali su koji se dugo godina koriste za obnovu koštanog tkiva.
Trombocitima obogaćena plazma (engl. platelet rich plasma, PRP), trombocitima obogaćen fibrin (engl. platelet rich fibrin, PRF) i plazma bogata faktorima rasta (engl. plasma rich in growth factors, PRGF) preparati su dobiveni centrifugiranjem krvi pacijenta i predstavljaju optimizirani krvni ugrušak. Upotrebom trombocitnih preparata može se pospješiti proces cijeljenja poticanjem neoangiogeneze, proliferacije fibroblasta, stvaranja kolagena, mitoze mezenhimalnih matičnih stanica i osteoblasta. Dok učinak PRP-a na koštanu regeneraciju još uvijek nije točno definiran, PRGF i PRF nalaze široku primjenu u oralnoj kirurgiji.
Od njihovog otkrića 1965. godine, koštani morfogenetski proteini (engl. bone morphogenetic proteins, BMP) privlače pažnju i predmet su brojnih istraživanja. Zbog pleotropne naravi ovih faktora rasta, osim izvrsnih rezultata u poticanju regeneracije koštanih defekta, njihova primjena nosi i određene rizike. Potraga za idealnim nosačem koji bi omogućio primjenu BMP-a u nižim koncentracijama još uvijek traje.

Abstract (english)

Bone possesses exceptional ability for regeneration so most injures heal without scar tissue. However, different local and systemic factors can compromise bone healing and result in bone defects. In those situations, surgical methods are used to promote bone regeneration.
Autogenous and allogenic bone, xenografts and alloplastic materials alone or in combination with membranes in guided bone regeneration technique (GBR) have been used for many years to restore bone tissue.
Platelet rich plasma (PRP), platelet rich fibrin (PRF) and plasma rich in growth factors (PRGF) are supplements obtained by centrifugation of the patient’s blood, representing an optimized blood clot. The use of platelet concentrates can improve the healing process by stimulating neoangiogenesis, fibroblast proliferation, collagen formation, osteoblast and mesenchymal stem cell mitosis. While the effect of PRF on bone regeneration is still not precisely defined, PRGF and PRP are widely used in oral surgery.
Since they were discovered in 1965, bone morphogenetic proteins (BMP) have been attracting a lot of attention and were subjects of numerous studies. Due to their pleotropic nature, besides excellent results in inducing bone defects regeneration, their use carries certain risks. The search for an ideal carrier which would allow the use of BMP at lower concentrations is still ongoing.
