Abstract

Trudnoća je jedinstveno vrijeme u životu žene, popraćeno raznovrsnim fiziološkim, anatomskim i hormonskim promjenama koje mogu utjecati na pružanje oralne zdravstvene zaštite. Razumijevanje tih uobičajenih promjena nužno je za pružanje kvalitetne njege trudnica. Međutim, trudnice kao pacijenti nisu medicinski ugrožene i ne bi trebalo odgađati provođenje potrebnog dentalnog tretmana.
U trudnoći se javljaju pojedine promjene stanja zubi i okolnog tkiva usne šupljine, ali se one većinom mogu spriječiti ili kontrolirati pravilnom prehranom i provođenjem pravilne oralne higijene. Uloga stomatologa je da trudnici naglasi važnost prevencije tih promjena, poput gingivitisa, parodontitisa, povećane pomičnosti zuba, epulisa, karijesa, dentalne erozije i ptijalizma.
Odontogenu upalu, kao i hitna bolna stanja, treba pravilno sanirati bez obzira na stadij trudnoće. Radiološka snimanja treba maksimalno izbjegavati, pogotovo u prvom tromjesečju. Potreban je poseban oprez prilikom primjene lijekova u trudnica zbog toga što većina prelazi placentalnu barijeru i može štetno utjecati na fetus. Američka agencija za hranu i lijekove podijelila je lijekove u pet kategorija od kojih svaka označuje utjecaj lijeka na trudnicu i fetus.

Abstract (english)

Pregnancy is a unique period in a woman's life, accompanied by a variety of physiological, anatomic and hormonal changes that may impact the oral health care delivery. Understanding these common changes is necessary for providing high-quality maternal care. Nonetheless, pregnant women as patients are not medically endangered and necessary dental treatments should not be postponed.
Even though certain changes related to teeth structure and surrounding oral mucosa occur during pregnancy, they can mostly be prevented or controlled by proper nutrition and by maintaining a proper oral hygiene routine. The dentist's role is to explain the pregnant patient how important it is to prevent the potential changes, such as gingivitis, periodontitis, increased tooth mobility, epulis, caries, dental erosion and ptyalism.
Odontogenic inflammation, as well as painful conditions, should be properly treated regardless of the stage of pregnancy. Radiological imaging should be avoided, especially in the first trimester. Special caution is required when administering drugs to pregnant women because most of the drugs cross the placental barrier and may adversely affect the fetus. The American Food and Drug Agency has divided the drugs into five categories, each of which indicates the effect a drug has on pregnant women and the fetus.
