Abstract

Iako su fiksnoprotetske konstrukcije najugodniji protetski radovi, velika većina parodontno ugroženih pacijenata nema uvjeta za njihovu realizaciju. Stanje zuba i njihov raspored često je nepovoljan za isključivo fiksne protetske radove pa je češća kombinacija fiksnih i mobilnih nadomjestaka, no prije izrade bilo kakvog protetskog nadomjestka važno je da stanje parodonta bude na zadovoljavajućoj razini jer o njemu ovisi vijek protetskog rada. Svaki zubni nadomjestak opterećuje uporišne zube, pa je njihov parodont izložen ne samo normalnom žvačnom nego i dodatnom opterećenju zbog nadomiještenih živčanih jedinica. Negativne posljedice opterećenja prije će doći do izražaja u oštećenom parodontu nego u zdravom. Definitivna protetska opskrba može se planirati i provoditi tek nakon saniranja parodonta. Loše isplaniran i/ili napravljen protetski nadomjestak može štetno djelovati na parodont. Stoga, mostovi i djelomične proteze moraju biti isplanirani i napravljeni tako da ne uzrokuju nova oštećenja parodonta. Posebnu pozornost treba obratiti na rubove krunica, konturiranje zuba i aproksimalnih dodira jer ako su oni nepravilno izrađeni pospješit će se odlaganje plaka, što će opet dovesti do upalnih promjena gingive.

Abstract (english)

Although fixed prosthodontic constructions are the most comfortable ones, the majority of patients with periodontal risks do not meet the conditions for their use. Often the condition of teeth and their setting does not favour fixed proshodontic appliances. A combination of fixed and removable appliances is, therefore, more frequent. Before a proshodontic appliance is made, it is very important that the condition of the periodontium is of satisfactory quality as the functional and aesthetic durability of a prosthodontic appliance depends on it. Every prosthodontic appliance puts strain on abutment teeth so that their periodontium is exposed not only to normal mastication forces but also to additional load because of the prosthodontic mastication forces. The consequences of excesive loading will become noticeable much sooner in damaged than in healthy periodontium. Definitive prosthodontic appliance can be planned and implemented only after periodontal treatment. On the other hand a poorly manufactured prosthodontic appliance can have damaging effect on the periodontium. Bridges and partial removable prosthodontic appliances have to be made, therefore, in such a way that they do not cause further damage to the periodontium. Special care should be taken of the crown margins, the contouring of teeth and to approximal contacts because if they are not made properly they will encourage the formation of plaque, which will in turn result in inflammatory changes of the periodontium.
