Abstract

Bol ima funkciju upozoravanja na oštećenje tkiva te funkciju aktivacije obrambenih refleksa s ciljem prevencije daljnjeg oštećenja. Podražaj koji oštećuje ili prijeti oštećenjem tkiva aktivira nociceptore koji prenose informaciju sustavom neurona do kore velikog mozga gdje se obrađuje i interpretira kao bol. Većinu somatosenzornih informacija iz područja orofacijalnog sustava prenosi n. trigeminus. Unatoč mnogim spoznajama o anatomiji i fiziologiji pulpo-dentinskog kompleksa, mehanizam prijenosa podražaja od površine dentina do terminalnih živčanih vlakana, još uvijek nije u cijelosti razjašnjen. Danas ga objašnjavaju:

1. Teorija prijenosa
2. Hidrodinamska teorija
Brännströmova hidrodinamska teorija čini se najprihvatljivijom. Prema toj teoriji bol izazvana podražajem (termičkim, kemijskim, mehaničkim) posljedica je strujanja dentinske tekućine u tubulusima. Takvo strujanja podražuje mehanoreceptore i dovodi do inicijacije živčanog impulsa u subodontoblastičnom pleksusu što rezultira nastankom boli. Radi jednostavnosti i sistematizacije pri određivanju dijagnoze, dobro se koristiti klasifikacijom oralne i kraniofacijalne boli unutar koje se razlikuju slijedeće grupe uzroka boli: ekstrakranijalna, intrakranijalna, vaskularna, muskuloskeletna, neuralna, kauzalgična i psihološka grupa boli. U klasifikaciji su navedene i opće karakteristike boli te pojedine grupe uzroka koje će olakšati identifikaciju klase, nakon čega slijedi detaljan klinički pregled i testiranja s ciljem određivanja točne dijagnoze unutar pojedine grupe. Dijagnoza atipične odontologije uspostavlja se isključivanjem svih ostalih uzroka orofacijalne boli.

Abstract (english)

The purpose of pain is to serve as a warning of tissue damage and initiate withdrawal reflexes to prevent any futher damage. Noxious stimuli provoke nociceptors that transmit the information by a system of neurons to the cortex where it is modified with previous experience and interpreted as pain. The most somatosenzoric information from orofacial area is transmitted via n. trigeminus. In spite of numerous cognitions of the anatomy and phisiology of pulp and dentin, the mechanism of stimulus transmission from the dental surface to the terminal nerve fiber hasn´t been completly yet explained. Today, it is explainted by three theories:
1. The theory of direct stimulation of the nerve
2.Transmission theory
3.Hydrodynamic theory
The Brännström´s hydrodinamic theory seems to be the most acceptable. According to this theory the pain caused by stimulus (thermical, mechanical, chemical) is the result of the circulation of the dental fluid in tubules. Such circulation irritates the mechanoreceptores and leads to initiation of nerve impuls in subodontoblastix plexus what results in pain.
Out of need for simplicity and systematization during diagnostic proces sit is recommended to use a clasification of oral and craniofacial pain which differs these groups of causes: extracranial, intracranial, vascular, musculoskeletal, neuritic, causalgic and psychological group. The classification also consists of some general pain characteristic of every group with the purpose of identification of the correct class. A clinical examination and clinical tests follow in order to make the exact
diagnosis within the class. The diagnosis of atypical odontalgia is based on elimination of other possible causes of orofacial pain.
