Abstract

Implantološka terapija sve se češće upotrebljava u terapiji bezubosti. Kako raste broj ugrađenih implantata, tako se povećava i broj komplikacija. Dijabetes, pušenje, loša oralna higijena i bruksizam samo su neki od faktora rizika koji uzrokuju komplikacije. Komplikacije se mogu podijeliti na biološke i mehaničke, ovisno o prirodi komplikacije. Biološke uključuju probleme oseointegracije i periimplantitisa, dok su mehaničke vezane za sam implantat, abutment, vijak ili protetski rad koji se nalazi na implantatu. S obzirom na to da je bruksizam multifaktorijalna bolest koja zahvaća značajan dio populacije, implantološka terapija takvih pacijenata gotovo je neizbježna. Iako ne postoje znanstvene potvrde koje bi bruksizam svrstale u kontraindikacije implantološke terapije, potreban je oprez pri postavljanju implantata kod takvih pacijenata. Komplikacije koje uzrokuje bruksizam povezuju se s okluzalnim silama visokog iznosa koje bruksisti mogu stvoriti. Stomatolog bi trebao biti svjesan rizika te ih objasniti pacijentu. Ispravnim odabirom okluzalne sheme, broja implantata, izgleda implantata te protetskog nadomjestka, uz redovite kontrole i terapiju bruksizma, broj komplikacija se može svesti na manje razine.

Abstract (english)

Implantology is increasingly used in the treatment of edentulism. As the number of implanted dental implants grows, so does the number of complications. Diabetes, smoking, poor oral hygiene, and bruxism are just some of the risk factors that cause complications. Complications can be divided into biological and mechanical, depending on the nature of the complication. Biological complications include issues with osseointegration and peri-implantitis, while mechanical complications are related to the implant itself, the abutment, the screw, or the prosthetic work on the implant. Since bruxism is a multifactorial disease affecting a significant part of the population, implantology treatment for such patients is almost inevitable. Although there is no scientific evidence that categorizes bruxism as a contraindication for implantology treatment, caution is required when placing implants in these patients. Complications caused by bruxism are associated with high occlusal forces that bruxism can generate. The dentist should be aware of the risks and explain them to the patient. By correctly choosing the occlusal scheme, the number of implants, the design of the implants, and the prosthetic replacement, along with regular check-ups and bruxism therapy, the number of complications can be reduced to lower levels.
