Abstract

Propolis je pčelinji proizvod koji posjeduje brojne biološke učinke zbog kojih se kroz povijest ali i u današnje vrijeme često koristio u medicinske svrhe. Međutim, valja naglasiti da su gotovo svi učinci dokazani u in vitro studijama ili na eksperimentalnim životinjama. S druge strane, antigeni potencijal propolisa dobro je poznat i opisan je veliki broj alergijskih reakcija. Svrha ovog istraživanja bila je prikupiti podatke o osnovnim kliničkim karakteristikama oralnih alergijskih reakcija na propolis. U našoj studiji sudjelovao je 21 ispitanik (13 žena i 8 muškaraca) liječen zbog nuspojava upotrebe propolisa tijekom 2004. i 2005. godine. Dijagnoza oralne alergijske reakcije je postavljena temeljem kliničke slike i anamnestičkog navoda o topikalnoj upotrebi propolisa dok je u jedne ispitanice alergija na propolis dokazana metodom epikutanog testiranja. U istraživanju su korišteni podaci iz medicinske dokumentacije ispitanika kao i telefonsko anketiranje. Iz dobivenih rezultata može se zaključiti da se oralne alergijske reakcije na propolis učestalije javljaju u žena. Najčešće se manifestiraju kao erozivna upala koja zahvaća cijelu usnu šupljinu ali se mogu javiti i lokalizirano, na bilo kojem dijelu usne šupljine. Simptomi se najčešće javljaju 3-4 dana nakon početka upotrebe propolisa iako se mogu javiti i kasnije. Lezije se uspješno liječe topikalnim kortikosteroidima a u rijetkim prilikama potrebno je ordinirati sistemske kortikosteroide. Ovisno o opsežnosti lezija, vrijeme cijeljenja iznosi 6 do 20 dana. Zbog svega navedenog smatramo da se propolis ne bi smio koristiti za liječenje bolesti usne šupljine.

Abstract (english)

Propolis, a product of the honeybees, has been used from ancient history in medicine because of its biological properties. However, all studies have been performed in vitro or on experimental animals. On the other hand, antigenic potential of propolis is well known and many allergic reactions have been documented. The purpose of this study was to obtain information about clinical manifestations of oral allergic reaction induced by propolis. Our study included 21 patients (13 women and 8 men) treated for sideffects of propolis use during the years of 2004 and 2005. The diagnosis of oral allergic
reaction has been made according to clinical picture and anamnestic data about the use of propolis. In one patient allergic reaction has been proved by patch test. Data from patients' charts and the phone interviews were also used in our study.
From the results of our study we can conclude that frequency of oral allergic reactions to propolis is higher in women than in men. The most commom clinical picture is erosive stomatitis even though the erosions can be localised in any part of the oral cavity. The symptoms usually appear 3 to 4 days after the aopplication of propolis. The lesions are successfully treated by topical corticosteroids and some rare cases may require systemic therapy. Time of healing varies from 6 to 20 days depending on the size of the lesions. From the results of this study we can conclude that the oral diseases should not be treated with propolis.
