Abstract

Prirođene srčane greške strukturne su anomalije srca čija učestalost u živorođenih uznosi 0,8%. Dijele se na greške bez patološkog spoja između sistemnog i pulmonalnog optoka (bez šanta) i na one sa patološkim spojem, među kojima razlikujemo one s pojavom cijanoze i bez nje. Ranim otkrivanjem i liječenjem poboljšava se prognoza bolesti. Većina grešaka zahtijeva kiruršku korekciju što omogućuje normalizaciju funkcije i trajanja života. U školskoj dobi može doći do spontanog zatvaranja defekta atrijskog i ventrikulskog septuma. Kao posljedica smanjene otpornosti oralnog tkiva na lokalne faktore, kongenitalne srčane greške uzrokuju i niz promjena u usnoj šupljini. One su češće kod srčanih grešaka s cijanozom. Najčešće su promjene parodonta u obliku gingivitisa i
parodontitisa te pojava jakog karijesa. Za prevenciju ovih promjena od velikog su značaja održavanje pravilne i redovite oralne higijene. Urođene srčane greške u većini slučajeva zahtijevaju kiruršku intervenciju te zbog velike mogućnosti kolonizacije ožiljka bakterijama spadaju u skupinu rizičnih pacijenata. Prilikom krvavih stomatološko-kirurških zahvata u takvih pacijenata stomatolog na smije zanemariti profilaksu bakterijskog endokarditisa.

Abstract (english)

Congenital heart defects are structural anomalies of the heart, occurring in 0.8% of newborn children. They can be divided into defects without pathological joint between pulmonary and system blood stream (without shunt), and those with
pathological joint (with and without presence of cyanosis). If the disease is diagnosed and treated in the early stage, the prognosis will improve. Most of these defects require surgical intervention enabling better quality of and longer life.
During the school age there is a possibility of spontaneous closure of the atrium and ventricular defect. As a consequence of the lower immunity of oral tissue, congenital heart diseases cause a number of undesired for the prevention of unwanted dental alterations changes in the oral cavity. These changes are more common if cyanosis occurs. The most common alterations of the gums are gingivitis, parodontosis and advanced dental cavity. Proper oral hygiene is essential for the prevention of unwanted dental alterations. Congenital heart diseases, in most cases, require surgical intervention.
Because there is a possibility of bacterial colonization of scar tissue, people who have undergone this kind of surgery belong to the high infection risk group. During any dental procedure on these patients which involve bleeding, dentist must never
disregard the importance of prophylaxis of bacterial endocarditis.
