Abstract

Parodontitis je jedna od najraširenijih bolesti u populaciji. Zahvaća potporni aparat zuba, uzrokujući njegovu destrukciju i, u konačnici, gubitak zuba. Današnja terapija parodontitisa naglasak stavlja na regeneraciju. Cilj nije samo zaustaviti bolest, već i pokušati obnoviti njene posljedice. Modernizacijom stomatologije, danas je prisutna široka paleta kirurških regenerativnih metoda – vođena tkivna regeneracija (GTR), vođena koštana regeneracija (GBR), uporaba bioloških faktora. Emdogain spada u skupinu bioloških faktora. Riječ je o geloznome preparatu koji sadrži derivate proteina caklinskoga matriksa. Ti amelogeninski i neamelogeninski proteini fiziološki se produciraju tijekom razvoja zuba i smatra se da igraju ključnu ulogu u cementogenezi. Upravo u tome leži regeneracijski potencijal Emdogaina. Iako se Emdogain godinama već uspješno kirurški primjenjuje u liječenju rezidualnih džepova, novija istraživanja pišu o njegovoj potencijalnoj korisnosti u kombinaciji s nekirurškom terapijom. Takvim suvremenim pristupom smanjila bi se potreba za kirurškim intervencijama, što bi olakšalo terapiju i pacijentu i terapeutu. Ipak, stručnjaci su tek zagrebli površinu navedene teme. Činjenica je da postoji potencijal za uspješnost takve terapije, međutim, potrebno je još dugo vremena i velik broj istraživanja da bi se utvrdila njezina učinkovitost i povećala predvidivost.

Abstract (english)

Periodontitis is one of the most widespread diseases in the population. It affects the supporting tissue of the tooth, causing its destruction and, ultimately, the loss of the tooth. Nowadays, periodontal therapy emphasizes regeneration. The ultimate goal is not only to stop the disease but to try to restore its aftermath. With the modernization of dentistry, today, there is a wide range of surgical regenerative methods guided tissue regeneration (GTR), guided bone regeneration (GBR), and use of biological factors. Emdogain belongs to the category of biological factors. It is a gel substance that contains an enamel matrix derivative. These amelogenin and non amelogenin proteins are physiologically produced during tooth development and are considered to play a crucial role in cementogenesis. Precisely there is where the regeneration potential of Emdogain lies. Even though Emdogain has successfully been used surgically for years in the treatment of residual pockets, recent research has shown
its potential benefits when combined with non surgical therapy. However, experts have only scratched the surface of this topic. The fact is that there is a potential for success of such therapy, however, a long period and many more studies are required to determine its effectiveness and increase its predictability of outcome.
