Abstract

Razdoblje trudnoće i dojenja vrijeme je kada su majke posebno povezane sa svojom djecom preko posteljice ili majčinog mlijeka. To je razdoblje intenzivnog rasta i razvoja djeteta te sve što se događa u tijelu majke utječe i na dijete.
Trudnoća utječe na zdravlje usne šupljine i zuba, a bolest zubi i potpornih tkivi može imati negativan utjecaj na sistemsko zdravlje majke i djeteta.
U trudnoći se češće javljaju pojedine bolesti i stanja zubi te usne šupljine, poput gingivitisa, karijesa, dentalne erozije, epulisa, parodontitisa, povećane pomičnosti zuba i ptijalizma. Oni se većinom mogu spriječiti ili uspješno kontrolirati pravilnim provođenjem oralne higijene i pravilnom prehranom. Zato treba staviti veliki naglasak na preventivu bolesti i edukaciju trudnica te dojilja.
Zbog straha da ne naštete majci i plodu, mnogi terapeuti odgađaju zahvate do postnatalnog razdoblja ili izbjegavaju propisivanje nužnog lijeka, što može dovesti do nepotrebnih komplikacija i potrebom za agresivnijom, sistemskom terapijom. Zbog toga je važno na vrijeme i adekvatno provesti stomatološku terapiju.
Provođenje stomatoloških zahvata sigurno je i za majku i za dijete tijekom cijele trudnoće. Hitni se zahvati uvijek obavljaju odmah, bez obzira na razdoblje trudnoće, dok se elektivni ipak obično odgađaju do 2. tromjesečja koji je za dijete najsigurniji, a majku najugodniji.
Pri propisivanju lijekova trudnicama i dojiljama treba se voditi smjernicama o kategorijama rizika pojedinog lijeka i propisivati ih samo ako su nužni.

Abstract (english)

For a mother and her child, pregnancy and breast feeding months are the most symbiotic period, when are connected through the placenta or breast milk. This is the period of intense growth and development for the child. Everything that affects the mother’s body also affects the child. Pregnancy has a profound effenct on the teeth and oral cavity in general. Disease of the teeth or periodontal tissue can have adverse impact on the systemic health of the mother and the child.
Some diseases are more prevalent in pregnancy, such as gingivitis, caries, dental erosions, epulis, periodontitis, increased tooth mobility, and ptyalismus gravidarum. They can be managed or successfully cured by prescribing diets or adequate oral hygiene. The main focus should be on the prevention of disease and the education of pregnant and nursing women. Due to their concern for the mother or the child, doctors often postpone treatment until the baby is born or avoid prescribing necessary drugs, which can lead to the progression of the disease and the need for more aggressive procedures or systemic therapy. For this reason it is imperative that the necessary dental procedures be performed properly and on time. Dental procedures are safe both for the baby and the mother all throughout the pregnancy. Emergency procedures are carried out immediately; regardless of the pregnancy stage, whereas elective procedures are postponed to the second trimester in order to ensure safety for the baby and comfort for the expecting mother. Pregnant women and nursing mothers should be prescribed drugs only if strictly necessary, and such drugs should comply with the risk-based classification of drugs
