Abstract

Jedna od ključnih kliničkih faza fiksnoprotetske terapije postupak je pričvršćivanja nadomjeska
odgovarajućim materijalom za pričvršćivanje, odnosno cementom. Uloga je cementa
ispunjavanje prostora između indirektnog nadomjeska i izbrušenog zuba nosača te osiguravanje
stabilne retencije nadomjeska u zadanom položaju tijekom vremena.
Klinička istraživanja pokazala su da su nedostatak retencije i nastanak karijesa glavni uzroci
neuspjeha krunica i mostova. Na tržištu postoji mnoštvo različitih cemenata s različitim
svojstvima prilagođenim određenim kliničkim zahtjevima. Međutim, to kliničaru ujedno i
otežava odabir odgovarajućeg materijala. Uspjeh cjelokupne terapije može se ugroziti
nepravilnim izborom materijala za pričvršćivanje i/ili nepridržavanjem propisanih kliničkih
protokola. Stoga su poznavanje dostupnih cemenata, njihovih svojstava, tehnika cementiranja i
karakteristika gradivnih materijala za izradu nadomjestaka neophodni kako bi se osigurao
uspjeh fiksnoprotetske terapije. Adhezivni su cementi u suvremenoj fiksnoprotetskoj terapiji
doživjeli svoj vrhunac primjene zahvaljujući sve većem zagovaranju minimalno invazivne
stomatologije. Tako je poštedna preparacija zuba trend koji je sve zastupljeniji u restaurativnoj
stomatologiji i fiksnoj protetici. Mogućnost poštedne preparacije zuba, u skladu s oblikom i
veličinom defekta, najveća je prednost adhezivne u usporedi s konvencionalnom tehnikom
cementiranja.
Svrha je ovog rada prikazati teorijske osnove adhezivnog cementiranja, mogućnosti koje nudi
u fiksnoprotetskoj terapiji, njegove prednosti i nedostatke te opisati materijale i vrste
nadomjestaka koji se adhezivno cementiraju.

Abstract (english)

One of the most important clinical phases of fixed prosthodontic therapy is the procedure of
fixing the restoration with a suitable material, i.e. cement. The role of cement is to fill the space
between the indirect restoration and the abutment tooth, and to ensure stable retention of the
restoration in a given position over time.
Clinical studies have shown that lack of retention and tooth decay are the main causes of crown
and bridge failure. There are many different cements on the market with different properties for
specific clinical requirements to choose from. However, this also makes it difficult for the
clinician to select the appropriate material. The success of the overall therapy may be
compromised by improper choice of the cement and/or non-adherence to prescribed clinical
protocols. Therefore, having knowledge of the available cements, their properties, cementing
techniques and characteristics of restorative materials is necessary to ensure the success of fixed
prosthodontic therapy. As a result of ever-increasing advocacy of minimally invasive dentistry,
adhesive cements are growing in popularity in modern fixed prosthodontics. Thus, minimally
invasive tooth preparation is a trend that is increasingly present in restorative dentistry and fixed
prosthodontics. The possibility of sparing the tooth structure, and a defect-oriented tooth
preparation are the biggest advantage of adhesive compared to conventional cementation
technique.
The purpose of this paper is to present the theoretical foundations of adhesive cementation, the
possibilities it offers in fixed prosthodontic therapy, its advantages and disadvantages, and to
describe the materials and types of restorations that are indicated for adhesive cementation.
